Abstract


Background and PurposeColorectal carcinoma is one of the most common gastrointestinal manlignant tumor, which incidence is second only to the stomach and esophagus cancer, the differences of incidence in different areas is obvious, comparing with the more developed countries in the world,our country belong to the low hair area,but with the changing of the lifestyle, its incidence is increasing year by year. Colorectal carcinoma occur hiddenly in early stage,and its development is relatively slow,most of the patients who appear the obvious symptoms and see a doctor are already in middle-late stage.The prognosis of colon cancer are relatively good, statistics showing,after the radical surgery,the total five-year survival rate can be more than50%, and the five-year survival rate of the early diagnostic and postoperative patients can achieve80%above, and the postoperative late5years of survival rate is only about30%. So early diagnosis and early treatments on the prognosis of patients with colon cancer is particularly important. Bmi-1(B-cell specific Moloney murine leukaemia virus insertion site1, Bmi-1) gene is one important member of the PcG genefamily, it participate in the occurrence and development of various tumors cooperating with the c-myc genes which can play the synergy effects, and it is also related closely to the tumor’s infiltration and metastasis. MMP-9(metal matrix protease-9) is the major play of the metal matrix protease family, it can lead to the invasion and metastasis of tumor by means of destroying the IV collagen in the basement membrane and promting angiogenesis. This study use the immunohistochemical S-P method to test respectively the expression of Bmi-1protein and MMP-9protein in57cases of colon adenocarcinoma(CA), comparing the the expression in25cases of normal colon mucosa(NCM) and40cases of dysplasia adjacent to the tumor(DA), and discuss significance and the relationship of the expression level of theBmi-1protein and MMP-9protein and clinical-pathological factors in the colon adenocarcinoma.Materials and MethodsAll the57cases of colon adenocarcinoma surgical specimens were achieved from the first affiliated hospital of Zhengzhou University between2009and2010, none of the patients received chemotherapy and radiotherapy before the operation.Using the immunohistochemical S-P method to test the expression of Bmi-1protein in57cases of colon adenocarcinoma(CA),40cases of dysplasia adjacent to the tumor(DA) and25cases of normal colon mucosa(NCM).Using the immunohistochemical S-P method to test the expression of MMP-9protein in57cases of colon adenocarcinoma(CA),40cases of dysplasia adjacent to the tumor(DA) and25cases of normal colon mucosa(NCM).Statistical analysis:All the data were analyzed by SPSS16.0statistical package, the Chi-square test was used to compare the positive rates of the qualitative data, the spearman level correlation analysis was used to analyze the relation of two variables, the significant difference level was α=0.05.Results1. The positive expression rates of the Bmi-1protein in colon adenocarcinoma tissue, adjacent atypical hyperplasia tissue and the normal colon mucosa tissue was respectively56.1%(32/57),52.5%(21/40) and20.0%(5/25), and with a significant difference in group comparison(P<0.05). The expression level of Bmi-1protein in colon adenocarcinoma tissue and adjacent atypical hyperplasia tissue was obviously higher compared with that in normal colon mucosa tissue from each other, there was statistical significance(P<0.01), but the difference of Bmi-1protein’s positive expression rate between colon adenocarcinoma tissue and adjacent atypical hyperplasia tissue has no significance(P>0.05).2. The expression of Bmi-1protein in colon adenocarcinoma tissue had closely relationship with the lymph node metastasis, depth of invasion and tumor’s TNM staging, but had no relationship with the patient’s sex, age and tumor’differentiation degree(P>0.05). The positive expression rate of Bmi-1in colon adenocarcinoma tissue with lymph node metastasis77.8%(14/18) was obviously higher than those without lymph node metastasis46.1%(18/39)(χ2=5.002,P<0.05),the positive expression rate in deep infiltration tissue71.4%(20/28) was obviously higher than those with shallow infiltration41.4%(12/29)(χ2=5.224,P<0.05)and the positive expression rate in grade of Ⅲ-Ⅳ group72.0%(18/25)was obviously higher than those in Ⅰ-Ⅱ group43.8%(14/32)(χ2=4,549,P<0.05).3. The positive expression rates of the MMP-9protein in colon adenocarcinoma tissue, adjacent atypical hyperplasia tissue and the normal colon mucosa tissue was respectively72.5%(41/57),47.5%(19/40) and16.0%(4/25), and with a significant difference in group comparison(P<0.05). The expression level of Bmi-1protein in colon adenocarcinoma tissue and adjacent atypical hyperplasia tissue was obviously higher compared with that in normal colon mucosa tissue from each other, there was statistical significance (P<0.01), and the difference of Bmi-1protein’s positive expression rate between colon adenocarcinoma tissue and adjacent atypical hyperplasia tissue had significance(P<0.01).4. The expression of MMP-9protein in colon adenocarcinoma tissue had closely relationship with the lymph node metastasis, depth of invasion and tumor’s TNM staging, but had no relationship with the patient’s sex, age and tumor’differentiation degree(P>0.05). The positive expression rate of MMP-9in colon adenocarcinoma tissue with lymph node metastasis94.5%(17/18) was obviously higher than those without lymph node metastasis61.5%(24/39)(χ2=6.605,P<0.05),the positive expression rate in deep infiltration tissue89.3%(25/28) was obviously higher than those with shallow infiltration55.2%(16/29)(χ2=8.211,P<0.05)and the positive expression rate in grade of Ⅲ-Ⅳ group88.0%(22/25)was obviously higher than those in Ⅰ-Ⅱ group59.4%(χ2=5.696,P<0.05).5. The expression of Bmi-1and MMP-9in colon adenocarcinoma tissue had a positive relationship(correlation coefficient r=0.299,P<0.05).Conclusions:The positive expression rate of Bmi-1was increasing gradually in colon adenocarcinoma tissue, adjacent atypical hyperplasia tissue and the normal colon mucosa tissue, and had a close relationship with the depth of invasion, lymph node metastasis and tumor’s TNM staging.This could be prompted that the high level expression of Bmi-1played a role in the occurrence development, invasion and metastasis of the colorectal carcinoma.The positive expression rate of MMP-9was increasing gradually in colon adenocarcinoma tissue, adjacent atypical hyperplasia tissue and the normal colon mucosa tissue, and has a close relationship with the depth of invasion, lymph node metastasis and tumor’s TNM staging.This could be prompted that the high level expression of Bmi-1play a role in the occurrence,development, invasion and metastasis of the colorectal carcinoma.3. Both of the Bmi-1protein and MMP-9protein may promote the occurrence and development of the colon adenocarcinoma collaboratively.4. Detecting the Bmi-1and MMP-9together could have a definite clinical meaning in diagnosising and estimating the prognosis of the colon adenocarcinoma.
