Abstract


Background and purpose of esophageal cancer is one of the of the most common malignant tumor mortality of malignant tumors in fourth place, morbidity and mortality highest in Henan, serious harm to the physical health of our people, and therefore of esophageal the occurrence of cancer development mechanism (CDM) has become one of the hot. 1979, Senger was first reported by the malignant transformation of epithelial cells can secrete a phosphorylated protein, i.e., osteopontin (osteopontin, OPN). OPN is rich in aspartic acid and sialic acid residues, the secreted glycoprotein, contains a variety of functional domains, which contains a specific Arg-Gly-Asp sequence (RGD), through the desired sequence and its growth microenvironment - combining the extracellular matrix (extracellular matrix, ECM). Disintegrin and CD44 receptor binding, OPN mediated cell interaction with the matrix and intracellular signal transduction, and thus play an important role in tumor progression. In recent years, more and more evidence to suggest that upregulation of OPN in many tumor cells and tumor angiogenesis, invasion and metastasis, inhibition of apoptosis are closely related. With the epidermal growth factor (epidermal growth factor, EGF) discovery (1962), the human epidermal growth factor receptor (epidermal growth factor receptor, EGFR) complete cDNA (1984), as well as depth to the cell signaling network study (90 years), it has been recognized as a receptor tyrosine kinase super EGFR and its family members of one of the members of the family, in the corresponding ligand EGF stimulation by receptor autophosphorylation involved in cell growth signaling , proliferation, survival and differentiation of epithelial-derived cells play an important role in the regulation. In recent years, the correlation of EGFR oncogene display cells Src (c-Src) oncogene and EGFR not only having a high degree of homology and are overexpressed in many human tumors, suggesting that the EGFR and its family members having oncogene potential, closely related with cell transformation. The tumor cells mainly through autocrine and paracrine TGF-α, EGF; acting on the EGFR, to obtain a growth advantage, suggesting that the signaling system plays an important role in tumorigenesis, development. OPN and EGFR in the relationship between the development of esophageal squamous cell carcinoma and its interaction research at home and abroad reported uncommon. In this study, using immunohistochemical methods to detect OPN and EGFR expression in esophageal squamous cell carcinoma, discuss its clinical significance, and analysis of the interaction of both in the development of esophageal squamous cell carcinoma. Materials and methods. Select Jiaozuo City Hospital surgical January 1992 to August 2000 60 cases of surgical resection of esophageal specimens (same cases including cancer tissue and adjacent normal tissues of cancer other than 5cm) HE slice pathologically confirmed squamous cell carcinoma. All patients had not received radiotherapy or chemotherapy, and 6-18 months of follow-up. 2. Application of immunohistochemistry SP method detected 60 cases of esophageal squamous cell carcinoma and its corresponding adjacent normal tissue OPN and EGFR protein expression. 3. Statistical tools with SPSS10.0 and SAS statistical package statistical methods using x 2 test, Spearman correlation analysis, the Kaplan-Meier and Log-rank test timing Inspection Act, inspection level α = 0.05. 1.60 cases of esophageal squamous cell carcinoma, 48 cases the OPN stained positive in 80.0% (48/60); 60 cases of cancer and adjacent normal tissue, no expression of OPN; results showed positive expression rate of OPN in esophageal squamous cell carcinoma statistically significant differences between the cancer and adjacent normal tissue (P <0.01). In esophageal squamous cell carcinoma, OPN is mainly present in the cytoplasm of the tumor cells. The OPN expression multi diffuse distribution in the edge of the cancer nests less cancer nests. 2.60 cases of esophageal squamous cell carcinoma, 43 cases of EGFR positive staining in 71.7% (43/60); 60 cases paracancerous organizations in 11 patients with EGFR positive staining in 18.3% (11/60);, EGFR of positive expression rate statistically significant (P <0.01) in esophageal squamous cell carcinoma and adjacent normal tissue; esophageal squamous cell carcinoma, EGFR of positive particles are mainly located in the cytoplasm and nucleus of the tumor cells, surrounding matrix no Positive staining. 3.60 cases of esophageal squamous cell carcinoma, the positive expression rate of OPN group of well-differentiated, 65.5% (19/29), 93.5% lower than in the poorly differentiated group (29/31), the difference was between the two significance (P <0.01); lymphatic metastasis OPN positive expression rate was 97.1% (34/35) than those without lymph node metastasis group 56.0% (14/25), there was significant difference between the two ( P <0.01); TNM stage Ⅰ and Ⅱ group of OPN in positive expression rate was 67.7% (22/31), below Ⅲ,, stage Ⅳ 93.1% (26/29), the difference between statistically significant (P <0.05); the infiltration depth of T 1 T 2 group OPN positive expression rate was 73.5% (25/34), lower than T 3 T 4 group and 88.5% (23/26), the difference between the two was not statistically significant (P> 0.05). 4.60 cases of esophageal squamous cell carcinoma, the group of well-differentiated EGFR positive expression rate of 57.7% (15/26), lower than in the poorly differentiated group and 82.4% (28/34), the difference was between the two significance (P <0.05); lymphatic metastasis EGFR positive expression rate was 83.9% (26/31), than those without lymph node metastasis group and 58.6% (17/29), there was significant difference between the two ( P <0.05); TNM stage Ⅰ and phase group II EGFR positive expression rate was 51.7% (15/29), below the stage Ⅲ, Ⅳ group, 90.3% (28/31) the difference between statistically significant (P <0.01); the infiltration depth T T 2 group of EGFR expression was 48.0% (12/25), lower than T 3 T 4 group and 77.1% (27/35), and the difference was statistically significant (P <0.05) between the two. 5. 6-18 months of follow-up, 60 patients with ESCC delete lost contact three cases, OPN protein expression positive survival rate was 39.1% (18/46), below OPN protein expression was negative 72.7% (8/11 ), and application of the Kaplan-Meier survival comparison of the Log-rank test timing Inspection Act;, the difference was significant (P <0.05). 6. 6-18 months of follow-up, 60 patients with ESCC delete lost contact three cases, EGFR protein expression positive survival rate was 38.1% (16/42), 66.7% (10/15 lower than the negative EGFR protein expression ), and application of the Kaplan-Meier Log-rank test timing Inspection Act, the survival rate comparison, the difference was significant (P <0.05). 7. 60 cases of esophageal squamous cell carcinoma, OPN and EGFR expression analysis, co-expression of the positive group, 38 cases, co-expression of negative group of seven cases, OPN, EGFR-expression group of 10 patients, OPN-, EGFR expression group five cases, the two were positively correlated, the Spearman correlation coefficient r s = 0.333 (P <0.05). Conclusion 1. OPN was highly expressed in esophageal squamous cell carcinoma and its high expression to their degree of differentiation and clinical stage, lymph node metastasis, prognosis, prompted OPN may be associated with the occurrence and progression of tumors. 2. EGFR over-expression in esophageal squamous cell carcinoma, and its over-expression of their degree of differentiation, and clinical stage, lymph node metastasis, depth of invasion, prognosis, and suggest that EGFR overexpression in tumorigenesis and invasion, metastasis process. 3. OPN and EGFR protein expression in esophageal squamous cell carcinoma by statistical analysis of the correlation indicates that they may act synergistically in esophageal squamous cell carcinoma, the process of development.
