Abstract


1 Background and Objectives Barry Marshall (Barry J · Mashall) and Robin Warren (J · Robin Warren) 1983 discovery of Helicobacter pylori (Helicobacter pylori, Hp) since, Hp and gastric cancer has become increasingly attention. Recent studies have found that people generally have higher Hp infection in developing countries than in developed countries; also found that Hp infection is not only closely associated with digestive diseases and blood system diseases, coronary heart disease, ischemic cerebrovascular disease closely related. Especially in northern China and the adjacent Forest City, Henan Province, Huixian, Anyang and other places is the high incidence of esophageal and gastric cardia, although Hp infection and digestive diseases in general has been relatively clear, but with the esophagus and cardia relationship is not clear. The laboratory research and other academic studies have found: Henan high incidence of esophageal gastric cardia different parts of Hp infection, cardia gastric antrum Hp infection lower than the preliminary findings suggest that patients with positive Hp infection cardia, its upper esophageal basal cell hyperplasia lesions less, but the cardia and antrum Hp infection in the same individual esophageal epithelial cell proliferation conditions are unclear. This study further analyzed the low incidence area of ??esophageal cancer in different parts of the stomach and esophagus characteristics and Hp infection, gastric cardia lesions relations, to further deepen the Hp in the cardia, gastric infection and its effect on the esophagus, cardia, gastric epithelial lesions affecting understanding. 2 Materials and Methods 2.1 study 152 cases of the same individual esophagus, gastric cardia with high incidence of esophageal tissue from the residents in Henan asymptomatic screening endoscopy biopsy income, not taking any medication. Of which 117 cases of gastric biopsies, 105 cases of cardiac biopsies, 152 cases of esophageal biopsies. Low-incidence areas with gastric cardia tissue from Puyang outpatient endoscopy biopsy in 79 cases of patients, including gastric biopsy in 39 cases, 40 cases of cardiac biopsy. 2.2 Methods All organizations have been 85% ethanol-fixed, paraffin-embedded, continuous 5um sections stained with HE histopathology. According to cell morphology, structure and degree of differentiation, the esophageal epithelium into normal epithelium (Normal, Nor), basal cell hyperplasia (Basic cell hyperplasia, BCH), atypical hyperplasia (Dysplasia, Dys); cardia and antral mucosa divided into normal epithelium (Normal, Nor), chronic superficial gastritis (Chronic superficial gastritis, CSG), chronic atrophic gastritis (Chronic atrophic gastritis, GAG), atypical hyperplasia (Dysplasia, Dys). With modified Giemsa (Giemsa) method for H. pylori was observed. Slice each case random observations 10 high power fields, according to the observed H. pylori were graded. Three positive points. The experimental results using statistical software SPSS10.0 exact probability method, correlation analysis using Spearman rank correlation, test level p = 0.05. 3. Results 3.1 high incidence of esophageal same individual cardia, gastric Hp infection and esophageal and gastric cardia lesions Relations cardia Hp infection (10%) was significantly lower than the gastric antrum (59%) (p <0.05). With the aggravation of Hp infection, antral lesion detection rate gradually increased. However, the cardia and antrum Hp infection and between different age and gender were no significant differences. Particularly noteworthy is the negative cardia Hp infection in patients whose upper esophageal basal cell hyperplasia and dysplasia detection rate was significantly higher than patients infected with Hp positive and Hp infection in gastric antrum was not observed similar results. But with age, Hp infection upward trend (p <0.05). 4 Conclusion 4.1 cardia low-incidence area of ??esophageal cancer Hp infection was significantly higher than the high incidence and high incidence of infection in the same individual cardia Hp-positive patients, the esophageal epithelial lesion detection rate is low, suggesting that Hp infection may cardia of the esophagus epithelial protective effect. 4.2 esophageal low-incidence areas below the high incidence of gastric Hp infection sites, however, Hp infection and gastric antrum was no correlation between esophageal epithelial lesions. 4.3 low-incidence areas of esophageal cancer increased with age, Hp infection positive rate tended to increase, but the high incidence similar results were not observed.
