Status of Health and Quarantine in Changshu and Management Countermeasure

Objective: To study the status of the entry-exit people infected with infections diseases, and the imported medical vector at Changshu Port. Put forward feasible and operable countermeasures and proposals for reducing the risk of the infectious diseases to a minimum.Methods: Reorganize and analysis the monitor data of the entry-exit peoples’infectious diseases in Changshu region from January 2005 to December 2009, and of all the containers from which medical vectors were intercepted at Changshu Port from January 2007 to December 2009, to look for the basic laws of the entry-exit prohibited infection diseases, and of the imported medical vectors’changes in categories, quantity and distribution. By referring the relevant literature, understand the current status of the monitoring work about the entry-exit people’s infectious diseases and the control of the imported medical vectors in inspection and quarantine system in our country.Results: The numbers of infectious disease surveillance of entry-exit people at Changshu Port are 1758 a year, mainly are laborers to leave the country, accounting for 53.81 percent more than five years constituted by the 2008 financial crisis, overseas labor service personnel from 2007 to 1107 people (53.3 %) dropped to 358 in 2009 (accounting for 30.21%), decrease of 67.66%.During 2005 to 2009,among 8788 entry-exit persons,509 disease cases were positive,the detection rate was 5.79%.In which,451 cases were HBsAg positive(5.13%),43 cases were HCV-Ab positive(0.49%),9 cases were found to be active pulmonary tuberculosis(0.10%), 6 cases were syphilis positive(0.07%),and no HIV-Ab positive was found. The detection rates of HBsAg-positive, HCV-Ab-positive and syphilis in Hong Kong, Macao and Taiwan compatriots were highest. They were 13.14%、1.17% and 0.26%.The detection rate of active pulmonary tuberculosis in laborers was the highest. It was 0.17%. The immigration containers were main source of medical vectors at Changshu Port, accounting for 98.154%; intercepted medical vectors contained fly, cockroach, mosquito and rat, the fly accounted for 97.217%(97.410% in containers, 86.960% in ships); the containers of intercepted medical vectors distributed in 19 countries, in which the United States carried a maximum batch, accounting for 38.70%, this was followed by Spain (accounting for 15.33%) and the Netherlands (accounting for 12.08%). Taken together, the Immigration container intercepted medical vector highest number in August.Conclusion: At present, Changshu port health and quarantine institutions in infectious disease surveillance among Exit-entry Personnel and the control of medical biological vectors with respect to do a large number of works, obtained certain result, but still exist many weak links. We should firmly establish the concept of epidemic diseases, timely understanding of the domestic Gaiden infected epidemic dynamics, strengthen pair of entry-exit personnel especially in Hong Kong and Macao compatriots, workers of the health education, improve the common infectious disease prevention and health care and self protection consciousness. To strengthen the entry container monitoring, aggrandizement responsibility consciousness, increase on-site inspection and quarantine strength, at the same time, do good propaganda work, to raise public awareness of infectious disease understanding and concern.Changshu port health and quarantine institutions run by the health and quarantine core ability construction as the turning point, increase technical equipment investment, strengthen the construction of the professional team, strengthen the construction of specialized laboratory, improve the monitoring ability and level. To introduce risk assessment system, promoting quality management. To strengthen and port-related departments to contact and cooperation, formed a joint control, perfecting the prevention system. To improve the port health quarantine emergency early warning mechanism, improving public health emergency response capacity.
