Abstract


Objective: 1. To study the changes and effects of the matrix metalloproteinases- 9(MMP-9) in pediatric patients with asthma and bronchopneumonia. 2. To explore the changes of immunologic function and pathogenesis in pediatric patients with asthma and bronchopneumonia.3. To explore the correlation between MMP-9 and immunologic function in pediatric patients with asthma.Methods: 107 children were divided into three groups according to their diagnoses: 36 cases in the group of asthma, 36 cases in the group of bronchopneumonia with wheezing, and 35 cases in the group of bronchopneumonia without wheezing. Detected the serum levels of MMP-9 applying reverse-transcription polymerase chain reaction (RT-PCR), while T cell subsets (CD3~+、CD3~+CD4~+、CD3~+CD8~+、CD4/CD8、CD3-CD19~+、CD19~+23~+、CD4~+CD25~+、CD(16~+56~+)~+ applying Flow cytometry(FCM)were measured, immunologic cells (Neutrophil, Lymphocyte, Eosinophil) with blood routine done, compared with 30 healthy children.Results: 1. Among all the groups of the patients, the levels of MMP-9 were significantly higher than that in the healthy children groups(P<0.001), with the group of asthma highest, the group of bronchopneumonia with wheezing second; the comparisons between every two groups of the patients had statistical significanc(eP<0.001). 2. Among all the groups of the patients, the levels of Neutrophil had statistical significance(P<0.001)compared with that of healthy groups. That of bronchopneumonia without wheezing was higher, and those of bronchopneumonia with wheezing and asthma were lower; the comparisons between every two groups of the patients had statistical significance(P<0.001) except the comparable group of asthma and group of bronchopneumonia with wheezing. Among all the groups of the patients, the levels of Eosinophil were significantly higher than that in the healthy children group(P<0.05); the comparison between every two groups of the patients had statistical significance(P<0.001) except the comparable group of bronchopneumonia with wheezing and the group of bronchopneumonia without wheezing. In the group of asthma, the levels of Lymphocyte were higher than that in the healthy children(P<0.001), while that of bronchopneumonia had no more statistical significance than that in the healthy children group(P>0.05); the comparison between every two groups of the patients had statistical significance(P<0.05) except the comparison group of bronchopneumonia with wheezing and group of asthma(P>0.05). 3. Among all the groups of the patients, the levels of CD3~+、CD3~+CD4~+ had statistical significance ( P < 0.001 ) compared with that of healthy groups, the group of bronchopneumonia without wheezing was lower, the group of bronchopneumonia with wheezing was lowest, but the group of asthma was higher; the comparison between every two groups of the patients had statistical significance(P<0.05) except comparison group of bronchopneumonia with wheezing and without wheezing(P>0.05). In the group of asthma and group of bronchopneumonia with wheezing, the levels of CD3~+CD8~+ were lower than that of healthy groups(P<0.05); the comparison between every two groups of the patients had not statistical significance (P>0.05). In the group of asthma and group of bronchopneumonia with wheezing, the levels of CD4/CD8 were higher than that of healthy group(P<0.05); the comparison between every two groups of the patients had statistical significance(P<0.05). In the group of asthma and group of bronchopneumonia with wheezing, the levels of CD3-CD19~+、CD19~+23~+、CD4~+CD25~+ were higher than those of healthy group(P<0.05); in the group of bronchopneumonia without wheezing, the levels of CD3-CD19~+、CD19~+23~+、CD4~+CD25~+ were also higher than those of healthy group, but that of CD19~+23~+ had statistical significance(P<0.05), while those of CD3-CD19~+、CD4~+CD25~+ had not statistical significance(P>0.05). Between every two groups of the patients, the comparison of levels of CD3-CD19~+ had statistical significance(P<0.05)except the comparison in the group of asthma and the group of bronchopneumonia with wheezing(P>0.05). Between every two groups of the patients, the comparison of levels of CD19~+23~+had no statistical significance(P>0.05)except the comparison the group of asthma and the group of bronchopneumonia without wheezing(P<0.05). Between every two groups of the patients, the comparison of levels of CD4~+CD25~+ had statistical significance(P<0.001)except the comparison the group of asthma and the group of bronchopneumonia without wheezing(P>0.05). In the group of asthma and the group of bronchopneumonia with wheezing, the levels of CD(16~+56~+)~+were lower than those of healthy group(P<0.05); although that of the group of bronchopneumonia without wheezing was higher, there was not statistical significance(P>0.05); the comparison between every two groups of the patients had statistical significance(P<0.05). 4. In the group of asthma, the levels of MMP-9 were showed a negative correlation with that of CD3~+CD8~+(r=-0.511, P<0.05), positive correlation with that of CD4/CD8 expression (r=0.509, P<0.05), but there was no correlation with those of CD3~+, CD3~+CD4~+, CD3-CD19~+, CD19~+23~+, CD(16~+56~+)~+, CD4~+CD25~+(P>0.05). In the group of bronchopneumonia with or without wheezing, the levels of MMP-9 had no correlation with those of T cell subsets(P>0.05). 5. Among all the patients, the levels of MMP-9 had no correlation with those of Neutrophil, Lymphocyte, and Eosinophil.Conclusions: 1.Not only does MMP-9 involve in the pulmonary infection, but also it relates with wheezing pathogenesis; MMP-9 plays a critical role in the pathogenesis of asthma, as well as the reference index for occurrence of asthma.2. Exist disorder of immunologic function in children of asthma, including the activity of cellular immunity, hyperfunctioning of humoral immunity, inducing synthesis of IgE, initiating reaction of hypersensitivity. Immunologic function reduced in children of bronchopneumonia, showed that cellular immunity was lower in the group of bronchopneumonia without wheezing, disorder simultaneous in the group of bronchopneumonia with wheezing. Pathogenesis of wheezing closely relate to proportion unbalance of CD4/CD8.3. In asthma, high expression of MMP-9 closely connected with function disorder of T subsets.4. There were changes of immunologic cells in peripheral blood in patients of asthma and bronchopneumonia, but no correlation with MMP-9.
