Abstract


1Background:Wang Ying-lin, male, born in Beijing, descendant of a well-known family of Chinese medicine, is the fourth-generation successor to the "leading pediatrics physician" in Beijing. Professor Wang Ying-lin apprenticed with his grandfather Wang Zi-zhong and his father Wang Pengfei since childhood. Born in a family of distinguished heritage, he takes great enjoyment in Chinese medicine. Professor Wang also received a formal college education. He graduated from Beijing College of Chinese Medicine in the year of 1964 as a Chinese Medicine Therapy major. Professor Wang is now instructor of the third and fourth National Senior Chinese Medicine Experts’Academic Experience Continuity Society. I have the honor to follow the instructions of Professor Wang and profit greatly by learning from him in clinic for more than 3 years transcribing prescriptions. Professor Wang is a famous clinic pediatric expert with profound knowledge of Chinese Medicine. He’s been practicing Chinese medicine for more than 40 years and has collected plentiful clinical experiences.Exogenous fever is frequently seen clinically. Fever is a common symptom for children and is also the cause of many other pediatric disorders. It is a disease that greatly influences their quality of life. Children are at an age of fast growth, their bodies are still immature and their immunity system underdeveloped, so they are weak in disease resistance. Pediatric exogenous fever causes symptoms such as coma, convulsion or seizure, and may sometimes be life-threatening. Currently the development of antiviral drugs for pediatric exogenous fever in modern medicine is rather slow and their efficacy is not quite explicit. Overuse of antibiotics leads to an increase in drug-fast bacteria as well as a decrease in the immunity of children. The side effects bring a lot of harm to children’s health.Professor Wang Ying-lin forms unique and effective pediatric treatments based on his family learning, adoption of other physicians’experience, and years of extensive clinical practice. His unique methods of palate inspection are very helpful in syndrome differentiation and treatment, and can reduce the pain and injury caused by laboratory tests such as blood drawing and film shooting, etc. In treating children with fever, Professor Wang creates Qinghan Mixture based on the idea of clearing heat, nourishing yin and resolving phlegm. He "avoids applying herbs with hot and dispersing nature in order to protect their congenital qi". While reducing high fever, he avoids depositing too much sweat so as to reserve the yin. He also applies expectorant to prevent progress of the disease and reduce incidence of serious complications. His therapy is proved quite effective.2Objective:This paper aims to make an over-all summary of Professor Wang Ying-lin’s academic thoughts and clinical experience. It focuses on his unique inspection method and clinical experience concerning the treatment of pediatric exogenous fever. The research may hopefully provide a guide for later generation to learn from numerous Chinese medicine books and to achieve better treatment for the well-being of mankind.3Content:This paper explores the origin of Professor Wang Ying-lin’s academic thoughts and makes an over-all summary of his clinical experience concerning the treatment of pediatric exogenous fever. It’s intended, on the one hand, to elevate his academic thoughts; and on the other, to conduct a more in-depth research into Qinghan Mixture. Hopefully, the paper can realize a fuller clinic value of Professor Wang Ying-lin’s unique prescription.4Method:There are 3 partsPart 1 Origin of Professor Wang Ying-lin’s academic thoughts:Collect information concerning Professor Wang Ying-lin’s family history by exploring the course of medical practice of his great grand-father Wang Ren-ji, grandfather Wang Zi-zhong, his father Wang Peng-fei as well as the development of their academic thoughts. This paper retrieves Chinese medical classics and medical works in past dynasties to find out which physician has had the most influence on Professor Wang’s academic thoughts. Such exploration helps gain more insight into the origin of Professor Wang’s academic thoughts.Part 2 An over-all summary:Professor Wang adheres to the principle of synthesis of the four diagnostic methods and lays stress especially on inspection diagnosis. This paper explores the theoretical support and key points of inspection of the palate, of the filth on the scalp, and also of the palms. Professor Wang is adept at treating children’s internal problems such as pediatric respiratory disease (for example, pediatric fever, cough, asthma, and adenoids), digestive system disease (for example, pediatric diarrhea, constipation, bellyache, vomiting, and malnutrition), pediatric hyperactivity, and pediatric enuresis etc. He is also quite good at treating children dermatosis such as infantile eczema, pediatric surgery such as hydrocele of tunica vaginalis, and ophthalmology disease such as stye. With regard to the compatibility of prescription, he follows the principle of "holistic recuperation, consideration of both the spleen and the stomach, and supplementation or subtraction depending on the conditions".Part 3 Clinical research:A clinical observation is made on 123 cases of outpatient receiving treatment in our hospital from September 2008 to March 2011. Randomized grouping method is adopted. Qinghan Mixture based on the idea of clearing heat, nourishing yin and resolving phlegm is conducted as the test prescription. Adhering to the principle of evaluation based only on the same category, pediatric Heat-clearing Oral Liquid (Xiao Er Re Su Qing Kou Fu Ye, produced by Jilin Yizheng Pharmaceutical Group Co., Ltd.) is conducted as the control prescription.3 days is taken as a course of treatment, during which clinical observation is made and the results recorded.5Conclusion:There are 3 partsPart 1 Origin of Professor Wang Ying-lin’s academic thoughts:The academic thoughts of Professor Wang Ying-lin originate from his ancestral lore. With Chinese medical classics as his foundation, Professor Wang not only absorbs the thoughts of well-known senior Chinese medical physicians but also benefits greatly from his years of medical practice and research.Part 2 An overall summary:The academic thoughts of Professor Wang Ying-lin can be summarized as follows. He adheres to the principle of synthesis of the four diagnostic methods and lays stress especially on inspection. His unique method of inspection compensates for the inadequacy of inquiry and inspection of children. He creates a variety of prescriptions for cough and applies them in an orderly manner depending on the stages of the disease and constitutional features of the patient. In treating pediatric hyperactivity, he not only regulates and balances the liver, kidney, heart and spleen, but disperses the lung-qi as well. In treating constipation, inspired by the idea of "splenoasthenic constipation" from Treatise on Fevrile Diseases(Shanghan Lun), he inherits the prescription handed down from his family and applies five main elements, weed (liquorice), wood (pawpaw), cane (uncaria), fruit (pericarpium citri grandis), and earth (humus flava usta). The composition is used to activate the spleen and nourish the bowel for the purpose of restoring the movement of stomach and bowels. He treats malnutrition by invigorating the spleen, tonifying the kidney, and resolving food stagnancy. Highly desirable results are often attained. Vomiting can be attributed to the inactivity of spleen and stomach and the failure of stomach qi to descend. Treatment of the disease is focused on activating the spleen and harmonizing the stomach. Sanjunzi Decoction consisting of ageratum, tuckahoe, and atractylis ovata is often applied as original prescription, based on which supplementation or subtraction can be made. Bellyache of the children can mostly be attributed to coagulation of cold pathogen. Therefore, herbs that can warm yang and dispel cold, eliminate dampness with aromatics, or regulate qi to alleviate pain are often applied. In addition to the function of relieving spasm, aromatics with naphtha can also strengthen the contraction of smooth muscle, so it serves two-way regulating functions. Treatment of diarrhea in children differs from that in adults. Early adoption of herbs that can invigorate the spleen, arrest discharge, and warm the kidney to stop diarrhea helps restore gastrointestinal function of the patient without leaving any pathogen behind. The cause of adenoids in children is basically phlegm and stasis, the treatment for which follows the principle of nourishing yin, tonifying qi, invigorating the spleen, reducing phlegm, unblocking orifices, and eliminating stagnation.Treatment of pediatric enuresis is based on reinforcing the kidney qi, tonifying the spleen qi, dispersing the lung qi, and arresting enursis. Chinese ephedra has opening property; and gingko, closing property. A collective application of them can ensure a smooth dispersion of lung qi and proper opening and closing of the bladder. Dispersing the upper jiao and unclogging the lower jiao is just like lifting the lid to pour off water in the pot. It serves the function of improving the brain and arresting enursis. With regard to the compatibility of prescription, Professor Wang is inclined to apply herbs with mild nature and also at a low dosage, and using as few herbs in one prescription as possible. His prescription is reliable in efficacy, has little side effect, and is sold at a comparative price.Part 3 Clinical research:Results:Symptoms as well as body temperature before and after treatment are significantly different. According to effect evaluation criterion, the clinical cure rate reaches 76.2%, the apparent effective rate is 12.7%and the effective rate is 6.3%.The result proves the efficacy of Qinghan Mixture for pediatric exogenous fever, and is of certain value in the evaluation of Chinese medicine for treating fever. Discussion and analysis on behalf of Qinghan Mixture, a typical prescription represented by the idea of clearing heat, nourishing yin and resolving phlegm, the role of the components, and the application are made.6Significance:This paper has made a comprehensive summary of Professor Wang Ying-lin’s academic thoughts and clinical experience regarding his treatment of pediatric diseases. It may help realize a fuller clinical value of the treatment. Professor Wang adheres to the principle of synthesis of the four diagnostic methods and lays stress especially on inspection. Application of his method has greatly improved the therapeutic effect and enriched diagnostic methods for pediatric diseases.In treating pediatric exogenous fever, he lays stress not only on clearing heat, but also on invigorating the spleen and stomach and nourishing the yin liquid. An early application of phlegm reducing herbs and concurrent stress on both treatment and prevention help reduce the incidence of other sicknesses and progress of the disease. This paper provides more complete thoughts on the treatment of pediatric exogenous fever using Chinese medicine. Hopefully, it can help relieve the suffering of more pediatric cases and improve their quality of life.
