Objective: severe traumatic brain injury and stroke patients unconscious is a common symptom of neurosurgery. For such patients is still a lack of effective treatment, but also to maintain the life of such patients need costly, many scholars are pessimistic about their treatment. How coma after severe traumatic brain injury and stroke in patients with early waking increasingly become an increasingly prominent issue in the treatment of brain surgery. In recent years, scholars application of the median nerve electrical stimulation therapy in persistent vegetative state, achieved a certain effect. Abroad median nerve electrical stimulation has been used for the treatment of the acute stage of coma patients found to significantly shorten the duration of coma and reduce morbidity. According to the theory of Chinese medicine and acupuncture, the bilateral hands Jueyin through deep median nerve by acupuncture Pericardium by the relevant points can play an indirect role in the stimulation of the median nerve, and multiple acupuncture points to stimulate, increase treatment sites , also raise a single therapeutic dose, have a significant effect on cerebral coma awaking. Coma after severe traumatic brain injury and stroke patients continued for more than three months to enter a persistent coma the persistent coma awaking therapy at home and abroad gradually increased, although the reported cases have woken cases, but the overall effect is not exhaustive. satisfactory. Scholars woke treatment can reduce the chance of entering a persistent coma, increasing the possibility of such patients wake coma patients with early, comprehensive and promote. This study intends to explore the Pericardium through electrical stimulation of the cerebral coma wake-promoting effects and mechanisms. Methods: 40 patients with cerebral coma patients were randomly divided into a control group (n = 20), and stimulation group (n = 20) and control group given conventional drug treatment, including: the Xingnaojing 20ml 50g / L glucose saline 250ml, brain protein hydrolyzate 50ml 50g / L glucose saline 250ml, intravenously once / day. Stimulation group on the basis of conventional medication of bilateral hands Jueyin After electrical acupoint stimulation. Before the first treatment, patients in both groups via transcranial Doppler ultrasound (Transcranial Doppler, TCD) checks patients with vertebral - basilar artery peak systolic velocity (systolic velocity Vs) mean flow velocity (mean 'velocity , Vm). Immediately after the detection give stimulation group patients Pericardium by electrical stimulation of 30min, after stimulation in the same position, the same conditions downstream second detection. Immediately before and after contrast stimulate blood flow changes. Given daily hand the Jue Yin by electrical stimulation of 30min 10d for a course of treatment and rest after the end of a course ~~ 2d. Sustained electrical stimulation after a month, again to the control group and the stimulated group the same TCD indicators for the detection, and comparing the detected results. Compared before and after treatment Glasgow Coma Score (GCS), Glasgow outcome score (GOS), language function in the case of motor function and overall quality of life. Results: stimulation group patients given bilateral Pericardium after the relevant Point Acupuncture, vertebral - basilar artery blood flow velocity than before stimulation significantly improved instructions the Pericardium through electrical stimulation of the cerebral coma patients with vertebral - basilar artery blood flow velocity anomaly state with immediate effect. Continued to stimulate a month after the stimulation group patients vertebral - basilar artery blood flow velocity significantly improved compared to a month ago, the Pericardium by electrical stimulation of the cerebral coma patients with vertebral - basilar artery blood flow velocity anomaly obvious treatment effect. The stimulation group one month of treatment of cerebral blood flow velocity average higher than that in the control group. Stimulation group after 1 month of treatment wake up the number 11, the number of waking up in the control group of 8 people. GCS score, GOS score, the language function score, the overall quality of life score difference between the two groups was significant (P lt; 0.05). Motor function score difference was not significant. Conclusion: Pericardium by electrical stimulation than simply using conventional medication can enhance cerebral coma patients with vertebral - basilar artery blood flow velocity, is conducive to the restoration of the structure and function of the brain stem reticular drug treatment. Pericardium by the relevant points through the stimulation of the cerebral blood flow velocity becomes faster from slow, causing the increase in cerebral blood flow, improve the blood supply of the brain tissue, reducing the number of necrotic neurons and promote repose neurons returned to normal. Acupuncture Pericardium by electrical excitation can convey spinal cord anterior horn cell nuclear group on further excitement electrical conduction reticulospinal tract, spinothalamic toe caused by the excitement of the brain stem reticular formation and hypothalamus and other parts, thereby releasing unspecified inhibition of heterosexual uplink excited system, the cerebral coma patients awake or unconscious alleviate. The opponent Jueyin by the related points after electro-acupuncture stimulation, patients GCS and GOS score can improve to improve patient language function and quality of life. Have an active role in the treatment of for promoting severe traumatic brain injury and stroke caused by cerebral coma patients wake up.
