Abstract

Objective: According to the immunohistochemical ER, PR and Her-2 expression in breast cancer sub-type of breast cancer by immunohistochemical method typing anthracycline-containing neoadjuvant chemotherapy efficacy and prognostic significance. Methods: A retrospective analysis of Xiangya Hospital from January 2002 to December 2008 received 2-4 cycles CAF / CEF neoadjuvant chemotherapy and follow-up data can be obtained in 390 cases of breast cancer stage Ⅱ A - Ⅲ B patients data. Immunohistochemical results according to ER, PR and Her-2 breast cancer were divided into four kinds of molecular subtypes: luminal A-type (ER-positive and / or PR-positive and Her-2 negative), the luminal B subtype (ER-positive and / or PR-positive and Her-2 positive), Her-2 over-expression of type (ER-negative, PR-negative and Her-2 positive) and basal-like type (ER-negative, PR negative and Her-2 negative), and analysis of breast cancer The immunohistochemical method parting anthracycline-containing neoadjuvant chemotherapy and prognosis significance. Results: 1 whole group of 390 patients, using immunohistochemical retrospective data approximate molecular typing, luminal A type accounted for 42.3% (165/390), the luminal B type accounted for 13.3% (52/390), Her -2 expression type accounted for 20.0% (78/390), the basal-like type accounted for 24.4% (95/390) of various molecular subtypes in age, menopausal status, pathological type, clinical stage and histological grade There was no statistically significant difference (P gt; 0.05). Different molecular subtypes of breast cancer on the CAF / CEF neoadjuvant chemotherapy results were different. Luminal A type, Luminal B type, basal-like type, Her-2 overexpression type of CAF / CEF neoadjuvant chemotherapy in clinical efficiency were: 38.2%, 46.2%, 58.9%, 48.7%, basal-like type, Her-2 over-expressing the program reactive better than LuminalA type (P lt; 0.05, see Table 6-8), the whole group of patients received neoadjuvant chemotherapy subsequent surgical treatment and postoperative adjuvant therapy. 3, the end of December 2010, 390 cases in the whole group, complete follow-up of 325 cases, 65 cases were lost to follow dead lost to follow-up rate was 83.4% (325/390). Survival prognosis using the Kaplan-Meier method to analyze the various molecular subtypes, the basal-like type of disease-free survival (DFS), distant metastasis-free survival (DDFS) and overall survival (OS) curve is significantly lower than the luminal A type, luminal B type, Her-2 overexpression type of Basal-like subtype of disease-free survival, distant metastasis-free survival and overall survival minimum, the difference was statistically significant (P lt; 0.05). Conclusion: 1 sub-type of breast cancer by immunohistochemical method can predict patients with different subtypes of neoadjuvant anthracycline-containing chemotherapy sensitivity and prognosis of the treatment of breast cancer. Sub-type of breast cancer, according to the immunohistochemical ER, PR and Her-2 expression, plain and simple, practical, economic, and operating techniques, and more widely used in clinical.
