Abstract


Objective:To evaluate the accuracy and reliability of using Hirschberg Test and Krimsky Test for deviation measurement in children, which could help doctors get more precise understanding of these two methods and use them better in clinic.Methods:50 children were involved in this study, who were comitant horizontal strabismus. All these patient’s deviations were previously measured by alternate prism and cover test.4 experienced ophthalmologists were asked to estimate the angle of deviation on these children based on photographs of corneal light reflex position.In addition, they were asked to assess the Krimsky Test measurement of these patients who were photographed with varying amounts of prisms before their eyes. The ophthalmologists were asked if the prisms exactly corrected, overcorrected, or undercorrected the patients’deviations, which are according to the alternate prism and cover test outcomes. These 4 ophthalmologists were also asked to measured the patient’s deviations by Hirschberg Test in clinic at the same period. Finally statistical methods were used for data analysis. Results:1. With Hirschberg Test and Krimsky Test, each ophthalmologist evaluated the angle of deviation variously.2. The outcomes of Hirchberg Test by viewing pictures are more accurate and reliable than the outcomes of Hirschberg Test by using in clinic.3. When Kappa angle is negative, the outcomes of Hirschberg Test and Krimsky Test are more accurate.4. Outcomes by Krimsky Test are more accurate when angle of deviation is less than 50 prism.Conclusion:Both Hirschberg Test and Krimsky Test are imprecise methods for deviation measurement. The risk factors include the experience of ophthalmologists, the angle of deviation and Kappa angle. A more accurate method is needed to measure deviation for uncooperative patients with alternate prism and cover test.
