Abstract


In recent years, the impact of periodontal disease on systemic health gradually be taken seriously. An increasing number of epidemiological investigations, animal experiments and clinical studies have shown: periodontal disease and systemic many systemic diseases, such as diabetes, cardiovascular disease, premature birth and low birth weight children there are interrelated and reinforce each other, or can aggravate systemic disease progression. Periodontal disease is a chronic bacterial infections, as a possible risk factor for the occurrence and development of periodontal disease and preterm delivery and low birth weight children have begun to attract attention. However, actually exist between periodontal disease and preterm delivery and low birth weight children are interrelated and its possible mechanism is unknown. The study included animal experiments and clinical studies of two parts: a method of periodontal disease and preterm delivery and low birth weight children between the animal studies: 45 female SD rats were randomly divided into three groups, mild periodontitis group (A ), severe periodontitis group (B) and normal control group (C). Wire ligation rat experimental animal model of periodontitis. Normal after 8 weeks of pregnancy until delivery in rats. Pregnancy cycle observed three groups of rats, body weight and litter of newborn pups. Simultaneous detection of a rat peripheral serum TNF-alpha, PGE2 and MMP-9 level. Results: three groups of SD rats gestational age and weight of newborn pups was no significant difference (P gt; 0.05); and the average number of A, B two groups of newborn pups litter weight were lower than in group C (P lt; 0.05 ); group B average litter of newborn pups weight lower in group A (P lt; 0.05), but the A, B groups the number of newborn pups born was no significant difference (P GT; 0.05) ② A, B two groups of rats with peripheral blood TNF-alpha water average significantly higher than that of the control group C (P lt; 0.05); Group B TNF-alpha level was significantly higher in group A (P lt; 0.05); A, B PGE2 levels in the peripheral blood of two groups of rats was significantly higher than in group C (P lt; 0.05); PGE2 levels of A, B two groups no significant difference (P gt; 0.05) Comparison of the three groups of rats, MMP-9, P = 0.5788, the difference was not statistically significant. ③ three groups of rats gestational age at delivery and newborn pups weight and peripheral blood TNF-alpha, PGE 2 and MMP-9 level was no significant correlation (P gt; 0.05) Three groups of rats the number of newborn pups and litter weight in peripheral blood TNF-alpha and PGE2 levels were negatively correlated (P lt; 0.05). Three groups of rats the number of newborn pups and litter weight and peripheral blood of rats independent of the level of MMP-9. Conclusion: periodontal disease may be to some extent related to the reduction in the number of pregnant mice childbirth offspring and litter weight to reduce, and the more serious periodontal infection the number and average litter of newborn pups the lighter weight. Serum cytokines TNF-alpha, PGE2 increased may be pregnant rats an important mechanism for delivery pups reduce the number and litter weight loss (low birth weight). Periodontal disease and preterm delivery and low birth weight children the relationship between clinical research methods: Collect threatened premature labor in pregnant women, which preterm pregnancy between 28 to 37 weeks or with birth weight less than 2500g, standards-compliant 50 cases of pregnant women for the experimental group. Hospital full-term pregnant woman to be produced to the same period 50 cases in the control group. Maternity full-mouth periodontal examination record plaque index (PLI), calculus index (CI), gingival bleeding index (BI), periodontal probing depth (PD) and clinical attachment loss (CAL). Detection of two groups of pregnant women the peripheral serum inflammatory cytokines TNF-α, PGE2, MMP-9 and CRP levels. Results: preterm labor group (experimental group) compared with full-term pregnant women (control group) poor periodontal status, preterm pregnant women PLI, BI, CI and periodontitis prevalence rate was significantly higher (P LT ; 0.05); pregnant women, the PLI, BI, CI and of periodontitis prevalence of gestational age, birth weight was negatively correlated. Average Cytokine water in the peripheral blood of preterm labor group was significantly higher than that in the control group; cytokine levels in maternal blood PLI, BI, CI and periodontitis prevalence rate was positively correlated with gestational age, birth weight was negative correlation. Conclusion: periodontal disease and preterm low birth weight infants may exist between certain association. The impact of periodontal disease on preterm delivery and low birth weight children may be related to changes in inflammatory cytokines in peripheral blood.
