Abstract


Objective:to measure the plasma levels of YKL-40in COPDand control groups,then to study the clinic significance ofYKL-40in COPD.Methods:60exacerbation COPD patients,65stable COPDpatients and20healthy controls,from Jan.2012to Dec.2012,were included in this study.The plasma levels ofYKL-40,MMP-9,PCT,and CRP were detected by ELISA.Results:1．Levels of YKL-40were higher in exacerbation COPD andstable COPD groups than healthy group.2．In three groups,the levels of YKL-40were higher insmoking subjects, than nonsmoking subjects respectively.3． In exacerbation COPD group,there were negativecorrelation between YKL-40levels and FEV1%predicted,therewere positive correlation between YKL-40levels and MMP-9、PCT respectively.4．The serum YKL-40levels in poor clinical prognosispatients were higher than good prognosis patients in acuteexacerbation groups.5．In exacerbation group,the YKL-40and PCT levels were high er in patients with bacterial infection than no infectionpatients.Conclusion:1. Levels of YKL-40in exacerbation COPD and stable COPDgroups were higher than healthy subjects, Levels of YKL-40werehigher in smoking healthy control subjects,stable COPD andexacerbation COPD subjects than nonsmoking subjectsrespectively.2. Levels of YKL-40have positive correlation with MMP-9and hs-CRP means that YKL-40serve as a new target in thepathogenesis of COPD.otherwise, YKL-40has negativecorrelation with FEV1.0％pred, suggesting that YKL-40as aninflammatory factors can predict the severity degree of COPD.3．In acute exacerbation COPD group,The levels of YKL-40and PCT in bacterial infection subjects are higher than noninfection subjects. and the YKL-40levels in worse prognosispatients are higher than that with better prognosis patients.Suggesting that serum YKL-40level can predict the prognosisof COPD, but its reference value need further research.
