Correlation between CT Features of Localized Renal Cell Carcinoma and Prognosis

Abstract


Objective: To assess the value of CT features of localized renal cell carcinoma(LRCC) for predicting the outcome.Method:The CT features and follow-up results of132LRCC patients withtreatment of radical nephrectomy(RN) or nephron-sparing surgery(NSS) in ourdepartment from1998to2007were studied retrospectively.Results: For the132LRCC patients,89patients were treated by RN and43patients by NSS,1-year、3-year and5-year cancer-specific surviva(lCSS)were97.7%、93.9%and84.8%,respectively by the Kaplan-Meier statistical analysis. The T1tumorhad higher5-year CSS than that of T2(88.6%vs70.4%;respectively; p=0.020),The tumor necrosis had lower5-year CSS than that of un necrosis(52.4%vs90.9%;respectively; p=0.021),The Encapsulated tumor had higher5-year CSS than that ofnonencapsulated(96.2%vs77.2%;respectively; p=0.003).On univariate analysis, tumorsize (p=0.02)、tumor necrosis(p=0.007) and encapsulation(p=0.038) weresignificantly associated with decreased survival. The tumor calcification andinflammation did not affect tumor prognosis. Multivariat analysis showed that tumorsize(p=0.023) and tumor necrosis (p=0.031)were significantly associated withdecreased survival, Which proved to be poor prognostic factors for CSS. Theencapsulation was not significant predicitive factors.Conclusion: In summary, both the tumor size and tumor necrosis presented by CTScan proved to be poor prognostic factors of survival in LRCC. The tumorencapsulation was not clear factors for predicting the survival. In clinical, We should becautious about treatment of small、nonencapsulated and necrotic tumors.
