Objective:To observe the effect of encirlilng scleral buckling and simple scleral buckling surgery on ocular refractive parameters.Methods:We prospectively examined36eyes which diagnosed with rhegmatogenous retinal detachment (RRD),24eyes undergoing encircling scleral buckling operation and12eyes that were treated with scleral buckling operation. Pentacam system was used to determine the changes in corneal curvature and astigmatism before surgery and1week,4weeks,12weeks and24weeks after surgery. Axial length(AXL), anterior chamber depth (ACD) lens thickness(LH) and vitreous cavity length(VCL) were measured with A-scan ultrasonography before and1week,4weeks,12weeks and24weeks months postoperatively. All these datas were analyzed and compared pre-and post-operative.Results:In the6months after encircling scleral buckling operation,there were significant differences in axial length(AXL),anterior chamber depth(ACD),lens thickness(LH),vitreous cavity length(VCL),curvature and astigmatism of the central corneal surface anterior and posterior(Km and Astig),and the differences were statistically significant (p<0.05). With the passage of time, these changes gradually restore to the level of pre-operative, in the3m after operation, axial length(AXL),anterior chamber depth(ACD),lens thickness(LH),vitreous cavity length(VCL),curvature of the corneal surface anterior and posterior (Km)were basically stable, and the values above in the6m after operation were approached to the3m after operation. While in the1m after operation, astigmatism of the corneal surface anterior and posterior (Astig) was basically stable, and in the3m and6m after operation the value above was approached to the1m after operation. In the1w after scleral buckling operation, there were significant differences in axial length(AXL),anterior chamber depth(ACD),lens thickness(LH),vitreous cavity length(VCL),curvature of the corneal surface anterior and posterior(Km),and the differences were statistically significant (p<0.05). With the passage of time, these changes gradually restore to the level of pre-operation, in the lm after operation, the values above were basically stable, and in the3m and6m after operation the values were approached to the1w after operation. The astigmatism of the corneal surface anterior (Astig anterior) increased during3months post-operation, becoming stable in the3m after operation and the value in the6m approaching to3m post-operation. While the posterior astigmatism increased during lm post-operation, becoming stable in the lm after operation and the value in the3m and6m approaching to the1m after operation.In the1w,1m,3m,6m post-operation, after encirling scleral buckling operation, the change of were significantly comparing with simple scleral buckling operation, while the change of axial length(AXL),anterior chamber depth(ACD),lens thickness(LH),vitreous cavity length(VCL),curvature of the corneal surface anterior and posterior(Km) was significantly after scleral buckling operation comparing with encirling scleral buckling operation. Within6m after surgery, differences between the parameters of two kinds of operation mode decreases gradually, and maintain stability.Conclusions:Encircling scleral buckling operation and simple scleral buckling operation have short-term effect on ocular refractive parameters. The encircling scleral buckling operation has greater effect on biological measurement of eyes comparing with simple scleral buckling surgery. Simple scleral buckling surgery has greater effect on the astigmatism of posterior corneal surface than encircling scleral buckling operation. Comparing the effect on ocular refractive parameters, simple scleral buckling operation is better than that of encircling scleral buckling operation.
