Abstract


Objective: To assess the diagnostic value of detecting pulmonary embolism throughmonochromatic images of Gemstone Spectral Imaging and iodine content of perfusiondefects.Methods: Collected17patients who underwent CT angiography with spectral imagingmode and were diagnosed with pulmonary embolism. Images were reconstructed bytwo modes: mixed-energy mode, and70KeV mono-energy mode. Analyzed the imagesby using Gemstone Spectral Imaging Viewer. Separately recorded the presence andlocalization of the two modes of pulmonary embolism. Measured iodine content of theperfusion defects and the adjacent or contralateral normal perfusion district.Simultaneously collected20patients who underwent CT angiography with GE16rowspiral CT, and were diagnosed with pulmonary embolism, analyzed the images byaccompanied workspace, recorded the presence and localization of pulmonaryembolism.Results: The17patients who underwent CT angiography with spectral imaging modeshowed that:70KeV mono-energy mode images showed a total of102clots with trunk(n=9), lobar (n=15), segmental (n=57) or sub-segmental (n=21) distribution;mix-energy mode showed a total of83clots with trunk (n=9), lobar (n=16), segmental(n=37) or sub-segmental (n=21) distribution.20patients underwent GE16row spiralCT showed a total of103clots with trunk (n=7), lobar (n=16), segmental (n=63) orsub-segmental (n=17) distribution. There was significant difference (p<0.05) in thesegmental clots between the two modes of spectral CT. Iodine-based materialdecomposition images showed that: iodine content between normal lung parenchyma(17.350±6.906) and perfusion defects (5.217±7.425) was significant difference(p=0.000). Conclusion: Compared with mix-energy mode images,70KeV mono-energy modeimages improves the detection of segmental clots. Iodine-based materialdecomposition images show perfusion defects clearly, perfusion defect and quantitativeanalysis of iodine content contribute to the detection of micro-emboli of pulmonaryembolism.
