Abstract


0bjective Microalbuminuria (MAU) is indicative of renal damage. Recent data prove microalbuminuria is a marker of systemic vascular endothelial dysfunction and early target–organ damage in patients with primary hypertension. This research was to study the correlation between microalbuminuria and cardiovascular events in patients with primary hypertension.Methods Risk factors ,including gender , age, mean arterial pressure, course of hypertension , diabetes mellitus, smoking, body mass index (BMI), low-density lipoprotein cholesterol levels(LDL-C),and cardiovascular events ,including heart failure, acute coronary syndrome and stroke, were investigated on 122 patients with primary hypertension admitted between January 2010 and December 2010. Exclusion criteria include secondary hypertensive patients, Primary nephrotic patients and nephrotic patients caused by other reasons (diabetes excluded), patients with severe primary heart/brain disease, liver/ kidney disease, or endocrine/ hematopoietic disease, women of being pregnant or to be prepared to pregnancy, lactating women and psychopathic patients. The 24 hours urinary albumin excretion rates (UAER) of these patients were measured with immunoturbidimetry. According to the 24 hours urinary albumin excretion rates(UAER),the patients were divided into two groups:group A with normal albuminuria(UAER<30mg / 24H) , and group B with microalbuminuria(UAER 30~300mg/24H ).We adopted different statistic tests according to different observational indexes and data, such as t test for measuring data (to be exact, the t test of two independent-samples for measuring data), chi-square test for counting data, nonparametric tests for skewed distribution of data, logistic regression analysis for risk factors analysis. The descriptive form of normal distribution material is mean±standard deviation. P<0.05 was defined as statistical significance. Risk factors and cardiovascular events between two groups were compared.Results There is no statistical significance in risk factors between two groups(P>0.05), but group B has more cardiovascular events ,including heart failure, acute coronary syndrome and stroke(P<0.05).Multivariate analysis indicates microalbuminuria (MAU) is an independent risk factors of cardiovascular events.MAU and course of hypertension are risk factors of heart failure. MAU, BMI and course of hypertension are risk factors of acute coronary syndrome. Also, MAU is a risk factor of stroke.Conclusions Microalbuminuria can be considered as a independent predictor and risk factor of cardiovascular events.
