Objective: older sudden deafness is the age of the the sudden deafness elderly patients over the age of 60, its incidence is increasing year by year in recent years, its pathogenesis is more complex, combined with changes in physical and mental function in elderly patients, diabetic population, hyperlipidemia, hardening of the arteries associated with underlying diseases, increased blood viscosity, low microcirculation of blood vessel elasticity is more common, so difficult rehabilitation, less effective, and high morbidity. At present, China has become an aging population big country, the development of medical science has played an irreplaceable role in their physical and psychological recovery and improved quality of life. This topic retrospective analysis of the clinical data of our hospital with sudden deafness in elderly patients, to explore the relationship between risk factors, features and prognosis factors, analyzed the impact of its related factors on the incidence and treatment through its The observation of the effect, to explore a variety of related factors and prognosis of patients, to provide a reference for the elderly sudden deafness clinical prevention particularity. Methods: Department of Otolaryngology - Head and Neck Surgery, Luzhou Medical College Hospital from January 2002 to December 2009 admitted 440 cases with complete data cases of sudden deafness retrospective study on its incidence, age structure than onset incentives associated with symptoms audiological examination results and prognosis summarizes average 0.5 ~ 4kHz the Zhu frequency air conduction hearing threshold as Response Evaluation Criteria, divided the 60-year-old age group ≥ 60 years of age for the elderly group, lt; 60 years of age for the control group, comparative analysis of clinically relevant factors of patients with sudden deafness in old age group and the control group and the treatment effect. Application the SPSS13.0 software analysis system, the normal distribution of measurement data, expressed as mean ± standard deviation (x ± s), within-group analysis using t test; count data expressed as a percentage, the groups were compared using the X2 test. Inspection level a = 0.05, P lt; 0.05 is considered statistically significant difference. The results: 1, the number of visits and the proportion of the elderly group a yearly basis; spring and autumn high incidence; onset time for the early morning, accounting for 16.67%; incentives in the above sense or catching a history of more than accounted for 6.06%. 2, the degree of hearing loss in the elderly group of severe, very severe deaf-based (n = 55), patients with mild hearing loss only cases of ear average hearing threshold than the control group. And the hearing curve to flat and fall based mainly to the steep drop, down and gradually decreased type ear hearing curve. 3, the older group and the control group total efficiency has increased annually, both ears at the same time the incidence of poor efficacy. The age of the elderly group compared with the control group, the total effective rate was no significant difference (P gt; 0.05) and total cure rate was significantly different (P lt; 0.05), the older group, only 2 cases were cured. Efficacy and duration of disease of the elderly group and the control group a positive correlation (P lt; 0.05), and the duration of gt; 30-day treatment of 11 patients were invalid. Old age group hearing curve type and degree of hearing loss, with no significant difference in efficacy (P gt; 0.05). Old age group and the control group associated with tinnitus, ear choking feeling, dizziness and other symptoms and efficacy of unrelated (P gt; 0.05), in which the elderly group associated with efficacy in patients with type 2 diabetes compared with non-diabetic patients with significant difference (P lt; 0.05), the older group and the control group is associated with other diseases associated with efficacy unrelated (P gt; 0.05). 6, tympanometry, acoustic reflex threshold (acoustic reflex threshold, ART) is normal or not and effect relationship between the difference was not significant. Auditory brainstem response (Auditory brainstem response, ABR) test, the older group and the control group ABR check Ⅰ - Ⅴ waveform differentiation or not with the total effective rate was no significant difference (P gt; 0.05), and the cured and markedly The cases included in the total effective group to compare its old age group differentiation or not the difference of the total effective no significant sex (P gt; 0.05), control group Ⅰ ~ Ⅴ waveform differentiation or not exist and the total effective rate differences (P lt; 0.05). I inpatient treatment of patients with sudden deafness onset affected ear distortion product otoacoustic emission (Distortion product otoacoustic emissions, DPOAE) did not pass cured patients with end treatments troubles ear DPOAE rate of 14.75% (n = 9), which elderly patients DPOAE only through one cases. Conclusion: The incidence of elderly sudden deafness law similar to the control group, its incidence increasing year by year and the proportion of poor early morning onset up to slow the flow of blood, tissue perfusion; high incidence of viral infections higher prevalence in spring and autumn, and associated with the flu or a higher proportion of cases of cold incentives. , Senile sudden deafness is often associated with presbycusis, hearing loss was severe, the cure rate is low, both ears at the same time the incidence of poor efficacy However, with the improvement of the treatment, after treatment, the total efficiency has increased annually. 3, the cure rate of patients with sudden deafness and age of onset was negatively correlated, the greater the age, the lower the cure rate; shorter the course of the disease, there is always the higher the efficiency. 4 patients with sudden deafness accompanied by vertigo, tinnitus, ear choking flu symptoms with no difference in efficacy. With increasing age, the proportion of disease associated with significantly increased, these diseases may affect the inner ear vascular function and blood circulation, especially in the elderly group associated with efficacy in patients with type 2 diabetes worse. Tympanometry, acoustic reflex threshold determination of in assessing hearing impaired, some reference value. Ⅰ ~ Ⅴ waveform differentiation of the elderly group auditory brainstem response (ABR) test whether or not the total effective differences no significant, while the control group, there were significant differences, suggesting that the pathogenesis of elderly patients with sudden deafness may senile deaf the pathogenesis of similarities - Neural presbycusis. DPOAE may serve as sensitive indicators of early hearing recovery in sudden deafness. Treatment of senile sudden deafness should take a comprehensive treatment program that the main cause of treatment, particularly associated with the disease according to the physical laws of the elderly patients and various focus on early prevention; monaural ill, should take the initiative to intervene protection of healthy ears hearing.
