Abstract

ObjectsTo provide a set of evaluation tools, which is scientific, reasonable, quantitativecomparable, concise and easy to use, to manage and evaluate the fixed medical institutions.Then afford the evidence to the managers to establish the awards and punishment standards.Methods1. Questionnaire survey methodsEstablish the questionnaire of index system through Literature searching and expertinterview.2. Expert consultationThis study conducted two rounds of expert consultations; the aim of first round was toselect indicators, the second round was to select weight Settings of the index system.3. Analytic hierarchy process (AHP)To provide comprehensive evaluation of patient service of the fixed medical institution .Results1. Constructs an evaluation index system of the new type of rural cooperative medicalcare system.The evaluation system of the new type of rural cooperative medical care system wasdivided into three first-order index: hospital services providing ability, validity andrationality, and its subordinate 11 secondary indexes: the actual compensation, the actualcompensation than growth rates, the state’s basic drugs directory scale, un-reimburseexpense proportion, the average cost of the drugs in hospitalization, the growth rate ofaverage cost in hospitalization, hospitalization fees average growth rates, hospitalization feesaverage hospitalization days, average hospitalization days growth rate.2. Set up the weight of every evaluation indexUsing AHP method and combining with the advices of experts to determine thecorresponding indexes weights. First class index: there index’s weights were respectively0.1408, 0.6147 and 0.2445; the Secondary indexes: the weights were respectively 0.5083,0.1064, 0.0557 etc.3. Use holistic marking method to do an empirical research of the 34 fixed medical institutions in TaiyuanCollect the data of 32 sample hospitals in Taiyuan, calculated the respectively scores ofthe 34 medical institution by weighted addition and weighted multiplication. then rankingthem by the scores.Compare with the GI, the result shows that the average of senior hospitals is0.6977,higher than the score of middle rank hospitals 0.6422, by statistical test,t=2.286,df=30,P=0.029, there were significant differences. But between the comprehensiveand specialized hospitals, they were no significant differences(P=0.997).ConclusionConclusions1. In accordance with the management goals of the policy, applies hierarchy processestablishing an evaluation index system, divided them into three dimensions: respectivelywere hospital services providing ability, effectiveness and rationality. and chose 11indicators from these three aspects. By doing the reliability and validity inspect, show thatthe system were reliability and stable.2. Adopt the evaluation system to valuate the 34 fixed medical institutions, and theresults show that the system was stable and reliable. And the data were easy to collect,arrangement and calculation in practical; at the same time, the evaluation index system hasthe function of real-time management, allowing the executive improvement of the hospitals,were good to promote the improvement of comprehensive competitiveness.3. The index system designed by this study is suitable for evaluating different grade & kindof hospitals in Taiyuan in the aspect of patient service. As for the fixed hospitals in differentgrade, we need to classify them first then do the evaluation, but do not need to do this if theywere comprehensive or specialized hospitals.Originalities1. It’s the first time using AHP method to constructing index system for the new type ofrural cooperative medical care system. 2. For the first time, using a dynamic index as evaluation indexes, can urge the dynamicmonitoring management of the fixed medical institutions.
