Abstract

Purpose with the economic development of human society, the acceleration of the process of the improvement of living standards of the people and the aging population, chronic non-communicable diseases (hereinafter referred to as chronic diseases) has become one of the world's most pressing public health problems of the 21st century, to the world-wide people's health and life had a tremendous impact. Therefore, Governments and researchers in the world are seeking and the development of methods and strategies in response to the chronic. The fund raising is the key to resolving the problem of chronic disease prevention and treatment of chronic disease prevention, chronic disease prevention and control fund raising problems have been put on the agenda and has become one of the hot issues of concern among academics, but has been widespread domestic and international study of chronic disease prevention and control funding models weak links. In this study, using qualitative and quantitative research methods, summary and analysis of related papers published academic circles at home and abroad gathered on chronic disease prevention funding for China's specific national conditions and found that the existence of the problems and difficulties of our fund-raising process in chronic disease prevention and control, Finally, knowledge integration and knowledge into preliminary exploration suited to China's national conditions, chronic disease prevention and control of multi-channel, multi-level fund-raising mode. The method of this study is the use of the theory and methods of information science, economics, health services management, through theoretical analysis to determine the modes of financing of chronic disease prevention at home and abroad, a comparative study of the major funding model for prevention and control of chronic diseases in the different countries through knowledge integration method, and the Financing Situation Analysis of chronic disease prevention and control in China and the main problems and make policy recommendations. Main methods include: (1) literature; (2) consulting experts: through personal interviews, focus group discussions to obtain qualitative and quantitative data of this study need; (3) Knowledge Integration: draw on knowledge integration method to retrieve this all research literature on the literature for systems integration. 67 Results (1) eventually included in the literature, including English literature 43 Pian, 24 Chinese literature; case-control study of a literature design type, the remaining 66 documents from 2000 to 2010, the status of survey research and qualitative research. 22 literature study funding models for chronic disease prevention and control in China, including China's 14 cities, covering most areas of our country; 44 literature in the United States, Australia, Canada, Japan, Germany, India and other multiple Financing Situation of national prevention and control of chronic diseases, and compared with the situation in China and thus proposed financing models suitable for the prevention and control of chronic diseases in China. 5 Forum briefing included literature, there are four main American businesses, health organizations and associations, and other NGO's to raise funds for chronic disease prevention, and mentioned in an England NGO donations. (2) Based on the evidence of these studies, the main sources of financing for a variety of fund-raising mode share of proportion, fairness and Payment System Evaluation, come to Western developed countries, chronic disease prevention and control has been the formation of more stable funding mechanism, the basic can support the sustainable development of chronic disease prevention and treatment services; Overall its main funding channels government investment and medical insurance (including social Medicare and commercial health insurance), or a combination of both. In China, the mode of financing for major chronic disease prevention and control individuals at their own expense, and government investment in chronic disease prevention and control total financing proportion is too small, is far from meeting the needs of the chronic disease prevention and control; individuals at their own expense accounted for the proportion of the total financing of chronic disease prevention and control is too large, brought a heavy economic burden to the patient's family and social; chronic disease financing models due to the constraints of the economic level, social development, commercial health insurance in our country is temporarily unable to be fully realized; China's medical insurance mechanism is not perfect, and also take a very long specification and integration of the time; individuals, businesses and non-governmental organizations such as the role is not fully played out. Recommended (1) government investment: all levels of government and the CDC should pay attention to the prevention and care of chronic diseases, the prevention and control of centrally allocated funding based on existing resources; strengthen the government's financing responsibilities, the establishment of a special fund chronic disease prevention, and insist on government finances of chronic diseases prevention and control of fixed subsidies, at the same time to ensure the continuity and effectiveness of capital investment; emphasis on the rural and urban poor and other disadvantaged groups to improve the fairness of financing. (2) social health insurance: reasonable assurance as to the type of medical coverage chronic diseases, establish a reasonable fee compensation mechanism; accordance with the principle of a balanced budget, and gradually establish a separate funding mechanism. (3) commercial health insurance: Government actively guide business health insurance, to encourage residents to participate in the commercial health insurance; combined with health management organizations and social medical insurance institutions, and give full play to its functions. (4) individuals at their own expense: to develop a reasonable fee, the formation of a stable price management system; take between revenue and expenditure management mechanism, the establishment of a centralized, standardized expenditure management system. (5) Non-governmental organizations donated: learn from foreign experience, encourage community investment in chronic disease prevention and control, so as to broaden the financing channels; vigorously develop community chronic disease prevention funding channels, and combined medical insurance, increase the level of funding.
