Abstract


[Objective] To explore the hip, knee replacement surgery the presence of hidden blood loss; 2, to explore hip, knee replacement surgery occult blood loss factors; explore the clinical impact of hidden blood loss after hip replacement [Methods] 1, selected in September 2008 - March 2010 line of total knee arthroplasty (total knee arthroplasty, TKA) 32 cases of TKA group; underwent total hip arthroplasty (total hip arthroplasty THA) 54 cases for the THA group. Further divided into two subgroups in the THA group, including 26 cases of the use of bone cement prosthesis for bone cement group, and 28 cases of the biotype prosthesis non-bone cement group; based on height, weight, preoperative and postoperative hematocrit, Gross equation calculated recessive blood loss and related clinical indicators in the preoperative and postoperative detection. 2 THA and TKA groups hidden blood loss, the THA group use and the use of bone cement recessive blood loss, line arthroplasty men and women hidden blood loss and hidden blood loss and related clinical indicators Statistical analysis was performed. 3 May 2008 - July 2009, 71 patients underwent unilateral total hip arthroplasty using the Harris hip score system to follow-up (male 47, female 24 cases; aged 48 to 75 years old, an average of 68.3 years of age), the combination of hidden blood loss in patients undergoing analysis of the correlation between hip function recovery and occult blood loss. [Results] The actual blood loss total TKA patients averaged 1455ml, hidden blood loss of 694ml, a total of 48%; the THA patients hemorrhagic total average 1442ml hidden blood loss of 463ml of the total 32%; hidden blood loss in the two groups the difference was statistically significant. Further found that the use of bone cement group the uncemented between hidden blood loss was statistically significant by the the THA group of two subgroups. 2, the TTKA group serum free fatty acid (FFA) concentrations averaged 2365 umol / L, the THA group FFA concentration of 1626 umol / L, was positively correlated with the recessive blood loss and FFA. 3, statistically significant between the hidden blood loss and postoperative functional recovery, and the patient's body mass index, and age, gender, surgical limb was no significant meaning. [Conclusion] 1, in the presence of hidden blood loss in THA and TKA; uncemented occult blood loss more significant than the use of bone cement has. Between TKA and THA postoperative the recessive blood loss and serum free fatty acid correlation further concluded that fat embolism may be an important factor causing TKA and THA hidden blood loss after. 3, the recessive blood loss in total hip replacement surgery clinical impact, the recessive blood loss can be used as a reference index arthroplasty functional recovery.
