Abstract


Central venous catheter (CVC) is through the skin direct from the internal jugular vein, subclavian veins or femoral vein for puncture, along the blood vessels to inserted vena cava catheter directly. Central venous because its diameter coarse, blood flow speed, catheter inserted length relatively short, high success rate of catheterization, little affected by input liquid concentration and PH of the restrictions, input liquid were soon blood thinners, and won’t cause of hemal wall stimulation, little vascular complications, as for these advantages it has been widely applied in parenteral nutrition, chemotherapy, numerous transfusions, rehydration and central venous pressure is determined. Although central venous catheter have the above advantages, catheter will cause pneumothorax, hemorrhage, catheter related infections, thrombosis, complications not to be ignored. One of these catheter related infections has become a common cause of nosocomial infection, is to influence seriously op-patient cure rate of the important factors that also contribute to extend the length of time of the patients, increasing the main cause of hospitalization fees. So clarify the infections caused by various factors, and corresponding control measures to decrease the hospital infection, improve the success rate of the critically ill patient, and it has important theoretical and practical significance.We have come through preliminary studies on the influence of the catheter related infections risk factors, current clinical control catheter related infections measures mainly from strengthening aseptic manipulation of the risk of deeloping to reduce the infection, although it could have some effect, but still cannot reduce the incidence of catheter related infections fundamentally. A man is an organic whole, an infection is often the internal and external causes the result of comprehensive shaping, whether can from the overall Angle to take effective precautionary measures to reduce the incidence of infections, should remedy than infections occurred after the treatment effect and better. But how to accurately and rapidly screening catheter related infections high-risk groups and establishing suitable for such patients the pertinence foresight nursing scheme, and to test its in clinical application effect has not been seen.Objective This article from the overall prevention catheter related infections Angle, discuss in reducing nursing scheme implementation foresight central venous catheter related infections, to provide the scientific basis on catheter related infections.Methods Retrospective study of 69 cases by central venous catheter tube of the general patient information, laboratory results and complications after catheterization, and to analyze the impact of catheter-related infection risk factors, Under the guidance of clinical nurse specialist to develop preventive nursing methods. Including the high risk factors for catheter-related infections evaluation and dynamic quality management system, implementation of dynamic quality management and maintenance of tube sucked detailed track record in order to clearly understand the specific circumstances of each patient and the disease progression, identify problems and solved in time, observation and care purposefully, to reduce the incidence of catheter-related infection. Selected the clinical data from November 2009 to April 2010 75 patients with central venous catheters, With the random number method,75 patients were divided into experimental group 36 cases and control group 39 cases. implementation preventive nursing on the experimental group, Control group with routine catheter care, Understand the Implementation of the nursing programs in reducing central venous catheter-related infections in the role and effectiveness.Results Of catheter-related infections in patients with high risk factors including underlying diseases, their immune system, catheter site, catheter type, retention time and pathogenic factors. The CVC-RI rate of catheterization in experimental group is 5.56%, lower than the rate 15.38% in control group (P<0.05),The results has statistic significance.Conclusion With the application of preventive nursing ,It has effectively reduced the incidence of complications such as the infection, reduced hospitalization cost, improved the patients satisfaction and their quality of life. So we can have the conclusion that with the application of preventive nursing,The CVC-RI rate has reduced.
