Abstract


Objective : To investigate the need for pericardiocentesis and drainage of pericardial effusion in patients with clinical features to guide the diagnosis and treatment of these patients . Methods: 202 cases required pericardiocentesis and drainage of pericardial effusion in patients with clinical features, etiology and nature of pericardial effusion and compared . Results : In addition to 11 cases of acute myocardial infarction was too late for pericardiocentesis , the remaining patients were successfully pericardiocentesis and drainage. 202 cases of patients with pericardial effusion , transudate 12 cases , 179 cases of exudate . Exudate , tumor 76 cases , 63 cases of tuberculosis , iatrogenic in 18 cases, 15 cases of acute myocardial infarction , connective tissue disease in 10 cases. 76 cases of neoplastic pericardial effusion , lung cancer, 42 cases , seven cases of breast cancer , unknown primary tumor in metastatic adenocarcinoma in 6 cases, 6 cases of lymphoma , liver cancer four cases , three cases of mediastinal tumors , stomach , bladder, rectum cancer, malignant thymoma , rhabdomyosarcoma , melanoma, sarcoma of the pericardium and the heart of each one case of hemangioma . With neoplastic pericardial effusion group, tuberculous pericardial effusion younger (P lt; 0.05), a higher proportion of positive tuberculin test (P lt; 0.01), between the two groups in the heat , gender distribution , hemoglobin , erythrocyte sedimentation rate , blood lactate dehydrogenase , adenosine deaminase and pericardial effusion conventional , lactate dehydrogenase , adenosine deaminase no significant difference. 18 cases of iatrogenic pericardial effusion , tachyarrhythmia catheter ablation in 10 patients ( four cases of catheter ablation of atrial fibrillation , supraventricular tachycardia catheter ablation , 4 cases of idiopathic left ventricular tachycardia ablation two cases ) , percutaneous coronary intervention in 6 patients . Catheter ablation of arrhythmias iatrogenic pericardial effusion concurrent women significantly more than men , patients with atrial fibrillation catheter ablation concurrent pericardial effusion is higher than the non- atrial fibrillation patients, and 9 cases of catheter ablation underwent pericardiocentesis and drainage can be, I 9 cases need surgical repair of iatrogenic pericardial effusion . 15 cases of cardiac rupture after acute myocardial infarction patients during hospitalization were dead . Conclusion : the need for pericardiocentesis and drainage of pericardial effusion as exudate and bloody , cancer and tuberculosis as the main reason , but iatrogenic pericardial effusion and acute myocardial infarction can not be ignored , and actively pericardiocentesis and drainage is important rescue and treatment, but acute myocardial infarction complicated by acute pericardial effusion high mortality rate.
