Impact of Obesity on Clinical Efficacy of Laparoscopy-assisted Radical Gastrectomy for Gastric Cancer

Abstract


Objective To investigate obesity on laparoscopic -assisted gastric the radical technical feasibility and short-term effect of the impact . Method from May 2007 to June 2010 Affiliated Union Hospital of Fujian Medical Surgical Oncology , 531 patients with gastric cancer underwent laparoscopic -assisted gastrectomy , including body mass index ( BMI ) ≥ 83 cases of 25.0 kg / m ~~ 2 patients , called obese group , BMI lt ; patients of 25.0 kg / m ~~ 2 448 cases , known as non- obese . Comparing two groups of patients with surgery , postoperative recovery , conversion to open rate , complications and survival . The obese group operative time (224.7 ± 57.3) min, longer than the non- obese group ( 210.0 ± 57.9) min , and the difference was statistically significant ( P lt; 0.05 ) . The obese group blood loss, intraoperative and postoperative blood transfusion rate , postoperative day 1 WBC, neutrophil increase in the number of body temperature recovery time , anal exhaust time and postoperative hospital stay fairly with the non - obese group , the difference was not statistically significant (P gt; 0.05). The obese group, the average number of lymph node dissection (24.8 ± 8.4) gold / cases less than the non- obese group (29.9 ± 10.2) gold / cases , the difference was statistically significant ( P lt; 0.05 ) . Obesity group and non - obese group converted to laparotomy rate of 2.4% and 1.8% , respectively , the difference was not statistically significance ( P gt ; 0.05 ) . The incidence of postoperative complications of obesity group and non - obese group were 16.8% and 10.2 %, the difference was not statistically significance ( P gt ; 0.05 ) . The whole group of 502 patients ( 94.5%) patients 2 to 42 months of follow-up, survival curves of the obese group and non- obese group difference was not statistically significant ( P gt ; 0.05 ) . Conclusion Obesity extended laparoscopic-assisted gastrectomy surgery time , but does not affect the feasibility and safety , as well as short-term effect .
