Abstract


Objective: through regular ECG of patients with aneurysmal subarachnoid hemorrhage and myocardial injury markers measured observation of the relevant factors for myocardial injury . Methods: the period from July 2010 to January 2011 in Luzhou Medical College Hospital neurosurgery diagnosis of aneurysmal subarachnoid hemorrhage ( aSAH ) the parallel the head Lu Sanwei vascular imaging scans (CTA ) and / or digital subtraction cerebral angiography (DSA) confirmed the aneurysm and exclude patients with a history of cardiovascular disease . On the day of admission , the 3rd , 7th day regularly line ECG and cardiac injury markers measured quantitatively. Gender, age , Hunt-Hess score , Fisher score correlation factors using chi-square (Fisher exact ) test to explore the differences of ECG abnormalities among the different groups the incidence of myocardial injury markers elevated incidence . The correlation between the establishment of logistic regression to examine the above factors aSAH incidence of ECG abnormalities and elevated markers of myocardial injury multivariate regression analysis . Results : group of different gender , age group , Hunt-Hess score high and low groups , Fisher score high and low group after aneurysmal subarachnoid hemorrhage incidence of ECG abnormalities no statistically significant difference . Hunt-Hess score high and low groups between the , Fisher score high and low groups with aneurysmal subarachnoid hemorrhage sensitive troponin I ( TnI) , creatine kinase MB isoenzyme (CK-MB) increased significantly differences no significant difference in incidence of elevated myoglobin (MYO) . Logistic regression test showed that the above factors are independent risk factors for non - ECG abnormalities and elevated markers of myocardial injury after aneurysmal subarachnoid hemorrhage . Conclusions : 1 . Aneurysmal subarachnoid hemorrhage early Yi associated with ECG changes and myocardial injury markers TnL , CK- MB , MYO elevated , longer duration , but most reversible . Hunt-Hess grade and Fisher grade aneurysmal subarachnoid hemorrhage patients need routine the ECG markers of myocardial injury examination for early assessment of cardiopulmonary function , and timely care treatment .
