Abstract


Research purposes perioperative blood loss in surgery and total knee replacement surgery limb swelling extent full tourniquet in total knee arthroplasty midway tourniquet technology through research, tourniquet use in total knee arthroplasty choose to provide a reference. Prospective clinical observation research methods, and will meet the 40 cases included in the standard case, randomly divided into a treatment group (midway tourniquet) and control group (full tourniquet), eligible subjects 1:1 the proportion allocated to the treatment group, 20 cases, 20 cases of the control group were measured preoperative and postoperative days 1, 3, 7 lower limb circumference (soft tape measure suprapatellar under 10cm); preoperative, postoperative 1,3,7 days erythrocyte count (RBC), hemoglobin concentration (Hb) and red blood cell hematocrit (Hct), blood loss and postoperative drainage, and statistical analysis of the differences, so as to total knee arthroplasty used tourniquet choice to provide a reference. Results 1 postoperative blood cell analysis indicators compared with the preoperative reduce preoperative red blood cell count, hemoglobin concentration, hematocrit comparison showed a significant difference (P lt; 0.05) and postoperative days 1, 3, 7; 2 treatment groups using the tourniquet time was significantly less than that of the control group, after inspection t = 3.56, a statistically significant difference (P lt; 0.05); blood loss treatment group was significantly more than the control group, the difference was statistically significant (P lt; 0.05), the the postoperative drainage treatment group was significantly less than the control group, the difference was statistically significant (P lt; 0.05); compare two groups of perioperative blood loss, has been tested and t = 0.078, the difference was not statistically significant (P gt; 0.05); patients preoperative limb Week diameter was no significant difference, t-test P gt; 0.05, the difference was not statistically significant; after one day, three days, two groups of limb patellar last week and statistically significant (P lt; 0.05) compared with the preoperative and postoperative 7 days showed no statistically significant; postoperative day 1, the next week, three days the two groups limb patellar diameter preoperative comparison was statistically significant (P lt; 0.05); 4. postoperative day 1, day 3 suprapatellar swelling degree of comparative treatment group was significantly lower than the control group, t-test P lt; 0.05, the difference was statistically significant, 7 days after patellar degree of swelling was no statistical significance (P gt; 0.05); postoperative day 1, the degree of swelling in the three days infrapatellar compare the treatment group was significantly lower than the control group, t-test P lt; 0.05 , the difference was statistically significant, the degree of swelling after 7 days infrapatellar no statistical significance (P gt; 0.05). Conclusions midway tourniquet technology will not significantly increased surgical blood loss, at the same time reduce tourniquet time, reduce the tourniquet tissue, muscle and nerve compression, thereby reducing the neuromuscular injury, reperfusion injury and thrombosis concurrent The disease occurs the possibility of reducing the degree of limb swelling.
