Abstract


[Objective] To assess the efficacy and safety of Low intensity pulsed ultrasonography for fractures.[Method] According to Cochrane Systematic Review, electronic literature search without language restrictions of Medline (1966-2010.03), EMBASE (1966-2010.03), Cochrane Library (issue3,2010), CNKI (1966-2010.03), SCI (1974-2010.03), VIP (1966-2010.03) and CBM (1966-2010.03) from inception of the database to March 2010, and gray literatures had also been searched, furthermore, the bibliographies of retrieved papers and content experts were consulted for additional references. The quality of included studies was critically evaluated. Two reviewers independently agreed one ligibility; three reviewers independently assessed methodological quality and extracted outcome data. Data analyses were performed with The Cochrane Collaboration’s RevMan 5.0.1 software. Weighted mean difference (WMD) or standard mean difference (SMD) was calculated in continuous variables. For dichotomous data, odds radio (OR) or relative risk (RR) was determined. All effect values were expressed by 95% confidence intervals (CI). The evaluation criteria included time to clinical healing.[Result] Ten randomized controlled trials with total of 452 patients met the included criteria. The meta-analysis showed treatment with a low-intensity pulsed ultrasound signal can reduce clinical healing time compared with the placebo group [RR=1.43,95%CI (1.15,1.79) P=0.001], in group of non-operatively managed fresh fractures; [RR=1.73,95%CI(1.37,2.19)P<0.00001] in group of operatively managed fresh fractures; [RR=1.68,95% CI (1.24,2.28) P=0.0009] in group of operatively managed non-union. While compared with the placebo group the low-intensity pulsed ultrasound signal had no significant difference in group of non-operatively treated stress fractures [RR=1.01,95%CI (0.25,4.01) P=0.99].[Conclusion] Treatment with a low-intensity pulsed ultrasound signal can reduce clinical healing time, some trials included in the review were of poor quality. Multi-center, large-scale randomized controlled trials of higher quality are needed to support our results.
