Abstract


Objective: To evaluate the ways panreaticoenteric anastomosis of in pancreatic surgery. To study the safety and feasibility of a new method of pancreaticoenterotomy by pulling circular cone pancreatic duct into jejunumMethods: Eighty-four patients receiving pancreaticoduodenectomy between 1999 March and 2010 November were reviewed. The types of pancreaticojejunostomy included pancreaticojejunal end-to-end anastomosis, pulling naked pancreatic duct into jejunum and pulling circular cone pancreatic duct into jejunum.Result :1.The mean pancreaticojejunostomy operative time following pulling circular cone pancreatic duct into jejunum anastomosis was significantly lower than those of pancreaticojejunal end-to-end anastomosis. But there was no significant difference between pulling naked pancreatic duct into jejunum and pulling circular cone pancreatic duct into jejunum2. The average recovery time of gastrointestinal founction recovery following pulling circular cone pancreatic duct into jejunum anastomosis was significantly lower than those of pancreaticojejunal end-to-end anastomosis. But there was no significant difference between pulling naked pancreatic duct into jejunum and pulling circular cone pancreatic duct into jejunum3.The incidence of pancreatic leakage of pancreaticojejunal end-to-end anastomosis was 15.4%;The incidence of pancreatic leakage of pancreaticojejunal end-to-end anastomosis was 15.4%; The incidence of pancreatic leakage of the anastomosis of pulling naked pancreatic duct into jejunum was 11.4% There was no pancreatic leakage in pulling circular cone pancreatic duct into jejunum anastomosis. Conclusion: The procedure has a definite effect to avoid anastomotics leakage following panreaticojejunostomy and would be worthy of being recommended. But because Of the cases were so few that we need to done more works to prove the effect in the futurethe cases were so lack, the procedure would need be approved more in the future.
