Retrospective Analysis of the Influence of Portal Vein Thrombosis on Liver Function in Patients with Cirrhosis

Abstract


Objective:The highest prevalence of Portal Vein Thrombosis (PVT) is Portal Hypertension in cirrhosis. The pathogenesis of PVT is unclear and its impacts on liver function and prediction are not definite, so there aren’t any unified rules in the diagnosis and treatment for the disease. The aim of this study is to compare the liver function between PVT group and control group by retrospective investigation and hope to be able to provide some reference for clinical diagnosis, treatment and prediction of cirrhosis with PVT.Methods:1.Case selection:2500 inpatients with cirrhosis who were admitted to the Gastrointestinal Department of the First Affiliated Hospital of Dalian Medical University and the Affiliated Zhongshan Hospital of Dalian University from January 1998 to December 2008, of which 30 cases combined with PVT and 15 cases selected according to the following excluding standards were included in Group P;the control group was 26 inpatients with cirrhosis wihout PVT who were selected randomly according to the following excluding standards(Control group, Group C).2.Excluding standards:the patients had abdomenal surgery history(upper digestive tract sclerosis surgery,splenectomy ,portal-systemic shunt and portal-systemic hassab,etc),maligance and hematoproliferative diseases。3.The diagnostic standard of cirrhosis decompensation: the Clinical Diagnostic Criteria of Hepatic Cirrhosis in Viral Hepatitis Prevention and Control Program in 2000.4. The diagnostic evidence of PVT: comfirmed by epigastric enhanced CT and/or colourful dopplar ultrasound.5.Collected the data of ill courses, CHILD-PUGH score, and complica- tions(diabetes, cholecystitis, hypertension,coronary heart disease)after patients confirmed cirrhosis.6. SPSS software was used for Statistic analysis.Results:1.The etiology, ill courses and complications of the PVT group were not statistically different from the control group(P>0.05).2.The prevalence of PVT in cirrhosis was 1.2%; PVT with surgical and non-surgical history occupied 0.6% respectively, each accounting for 50% in cirrhostic patients with PVT.3.The liver function of the PVT group was not statistically different from the control group(P>0.05).Conclusion:By retrospective study of available data and comparing CHILD-PUGH scores of liver function, we concluded that cirrhosis with PVT didn’t worsen the liver fuction of cirrhostic patients.
