Abstract

Obejective Immuhistochemical SP method is used to detect the positive expression of VEGF-C,COX-2 in CSG,CAG,DYS and GC group, and to explore the relationship between their expression and the common clinicopathologic features of gastric cancer containing lymph metastasis. Meanwhile, the thesis aims to find out whether their overexpression could provide a theoretical basis and some auxiliary information for the prognosis of gastric cancer .First, the thesis makes a relative analysis of expression of VEGF-C,COX-2 in primary tumors and nodal metases from one case. Secondly, the thesis investigates whether expression of them would be a clinical indicator for lymph node status reliablely and effectively. Thirdly it examines the infection rate of Hp in all cases by to luidine blue staining, and the correlation and significance of the expressions of COX-2 and Hp. The aim of the paper is to show the practical significance of Hp eradication in both prevention and treatment of gastric cancer and provide fundamental basis for therapy of tumors with selective COX-2 inhibitors.Method 152 cases were obtained from Department of pathology ,the Affiliated Hospital of Yan’an University with complete clinical data during 2008 - 2009,among which 107 were biopsy specimens consist of 28 CSG, 25CAG , 23 mild DYS , 31 moderate and severe DYS ,while of which 45 were surgical specimens and their nodal metastases .Positive expression of VEGF-C,COX-2 in Nodal Metastases and Local Involvement of gastric carcinoma were examined by Immuhistochemical SP method ,then Hp infection of all cases were detected by toluidine blue staining .All datas were analyzed for significance using the statistical software SPSS 16.0 computer package.Chi-suare test was used in the field of enumeration data analysis. Result The positive expression rate of VEGF-C,COX-2 in 45cases of carcinoma was 57.7%,64.4%respectively.There was no significantly difference between various age-sexcomposition .The positive rate of two kinds of protein was also not correlated with the clinapathilogic fetures just like age ,gender ,differentiation,serosa invison ,metastasis and so on.Futher more,The positive expression of them was found higher in well-differentiated cases than poorly differentiated ones,but no significantly difference .Then a correlation between VEGF-C and COX-2 was found.Significant difference was observed in the rates of them positive immunostaining between the lymph node metastasis group and no lymph node metastasis group,the previous one was higher,and positive rates in lymph node metastasis group were higher than total positive rate in all carcinoma cases.The positive rate of VEGF-C in primary tumors were higher than nodal metastases,and the difference was statistically significant.There were no signifiacnt differences COX-2 expression between primary tumors and nodal metastases.Compared with the carcinoma group, the positive expression of COX-2 was significant higher in groups of precancerous lesion and CSG.There were higher positive rates in carcinoma than group of mild DYS , moderate and severe DYS,while there were no VEGF-C detected in both CAG and CSG groups. Compared to that the rate of Hp infection increased progressively from CSG to DYS , the rate of carcinoma were slightly low .It was demonstrated that Hp played an important role in progress of stomach inflammation aggravation and gastric carcinogenesis.The positive rate of cox-2 in carcinoma group which infected Hp was significantly higher than those without infection.,and the positive rate in carcinoma group was significantly higher than group of CSG, with infection..Totally,COX-2 over expression in group of stomach disorder which infected Hp,was higher than those without Hp.Conclusion Both VEGF-C and COX-2 is involved in lymph node metastasis of gastric carcinoma.Hp plays an important role in progress of stomach inflammation aggravation and gastric carcinogenesis. The mechanism of tumor lymph node metastasis is very complicated ,and the role of Hp in gastric carcinogenesis is not completely clarified,we shall hammer away at these problem till we set a solution.
