Background and objective:Cerebrovascular disease (CVD) is a highly disabling disease, and now the disease is lack of effective treatment , so focus on prevention. Now know that a single factor such as high blood pressure, high blood glucose and lipid metabolism disorder is clear cerebrovascular disease risk factors. This study focuses on the coexistence of these metabolic abnormalities, reflected in the metabolic syndrome with the incidence of acute cerebrovascular disease and prognosis, so as to provide reference to the prevention and treatment of stroke.Methods:1. study object: They are selected from Department of Neurology of the First Affiliated Hospital of Nanchang University from June to December in 2008, all patients suffer from the first acute cerebrovascular disease and confirmed by head CT or MRI ,and are in line with the development of cerebrovascular disease diagnostic criteria of the Fourth National Conference in 1995. cases of the control group about age, sex matched are non-medical peason of cerebrovascular disease.2. Collecting the clinical data, including: age, sex, occupation, waist circumference, blood pressure, blood glucose, total cholesterol, triglyceride, high-density lipoprotein, low density lipoprotein, lipoprotein(a), the signs of the nervous system about admission and discharge.3. Diagnostic criteria for metabolic syndrome is used the International Diabetes Federation (IDF) metabolic syndrome in 2005, a new definition. Nervous system marded by modified Edinburgh score - the Scandinavian Stroke Scale (MESSS)4. All data were analysised by SPSS11.5 software.Results:1. The incidence rate of Metabolic syndrome in patients with acute cerebrovascular disease is 30.70% (123/400),but in patients with cerebral infarction and cerebral blood group were 45.00% (90/200), 16.50% (30/200), the two groups compared,P <0.05,it has statistically significant difference; cerebral infarction and the control group 11.30% (9 / 80) compared, P <0.05 ,There were significant differences; cerebral hemorrhage and the control group 11.30% (9 / 80) compared, P> 0.05, there are have no statistical difference.2. The logistic regression analysis about Metabolic syndrome and related factors, the independent risk factors of cerebral infarction relate as follows: metabolic syndrome (OR = 2.832, P <0.05), hypertension (OR = 7.769, P <0.05), high blood sugar ( OR = 2.715, P <0.05) elevated triglyceride (OR = 2.453P <0.05), lower high-density lipoprotein (OR = 1.983, P <0.05); with intracerebral hemorrhage related to an independent risk factor for hypertension (OR = 12.136, P <0.05).3. The impact of ischemic stroke about different levels of Glucose in metabolic syndrome : Through the Logistic regression analysis ,the metabolic syndrome with diabetes, high blood sugar with the metabolic syndrome and normal glucose metabolism in the syndrome increase the risk of cerebral infarction, the OR value respectively are 9.24,3.69 and 5.35.4. The incidence of Metabolic syndrome in cerebral thrombosis group is 51.50% (67/130), the incidence of Lacunar infarct group is 32.90% (23/70), the two compared, P <0.05, it has statistically significant , the two respectively compared with the control group, P <0.05, it has statistical significance.5. The incidence of Metabolic syndrome in the anterior circulation infarction group is 43.50% (78/172), in the posterior circulation group is 42.90%, the two compared, P> 0.05, there is no significant difference ,the two respectively compared with the control group, P <0.05, it has statistical significance.6. The impact of varying degrees in Metabolic disorder with acute cerebrovascular disease, cerebral infarction group with three kinds of metabolic disorders is the highest rates of factors, for 67.80%, and the incidence of cerebral hemorrhage group to one kinds of metabolic disorders associated with factors or with factors of two kinds of metabolic disorders is the highest, respectively are 52.30% and 47.80%, comparison between the two, P> 0.05, it has not statistically significant.7. Cerebrovascular disease with Metabolic syndrome group compared with non-cerebrovascular disease group about the neurological deficit score when discharged and treatment , P <0.05, it has statistical significance.Conclude:1. Metabolic syndrome is an independent risk factor for cerebral infarction, with diabetes of metabolic syndrome is a greater risk factor , cerebral infarction associated with the remaining risk factors include hypertension, high blood sugar, high triglycerides, high-density lipid reduced protein.2. Metabolic syndrome has nothing to do with cerebral hemorrhage, hypertension.is the only independent risk factor for erebral hemorrhage3. Metabolic syndrome are related with cerebral thrombosis and lacunar infarction, in particular with the cerebral thrombosis.4. Metabolic syndrome in cerebral blood vessels before and after the cycle had no significant difference, the internal carotid artery system and vertebral - basilar artery system can be affected.5. More severe degree of metabolic disorder, the greater risk of suffering from cerebral infarction; but metabolic disorders is no significant correlation with the extent of the risk of suffering from cerebral hemorrhage, that is at the basis of hypertension increased high blood sugar, blood lipid disorders and other factors, does not increase the risk of cerebral hemorrhage risk.6. Combined with the simple patients of cerebrovascular disease, the short-term prognosis of the patients of acute cerebrovascular disease with metabolic syndrome is worse.
