Plasma Very Long Chain Fatty Acids Levels and Its Relationship with Insulin Resistance and Hyperandrogenism in Polycystic Ovary Syndrome Women

OBJECTIVES:To investigate the changes in plasma very long chain fatty acids (VLCFAs) in PCOS patients, and further to explore its relationship with insulin resistance and hyperandrogenism. A scientific basis for the prevention and treatment work for the PCOS patients is provided in our research.METHODS:Two hundred and five PCOS patients were screened out according to the diagnostic criteria of Rotterdam conference (2003), aged from 18-40 years old. A group of 117 healthy infertile women of corresponding period was taken as the normal control group. These women were chosen randomly from reproductive center due to the problems of uterine tube or males. Height (Ht), weight(Wt), waist circumference (WC), hip circumference (HC), triglycerides (TG), total cholesterol (TC), high density lipoprotein-cholesterol (HDL-C) and low density lipoprotein-cholesterol (LDL-C) were tested. The fasting plasma glucose (FPG) was detected by glucose oxidase.The plasma fasting insulin (FINS) and total testosterone(T) were examined by Chemoluminescence method. The plasma free testosterone(FT) was examined by enzyme-linked immunosorbentassay (ELISA). The BMI, waist-to-hip ratio (WHR),and homeostasis model assessment for insulin resistance (HOMA-IR) were calculated. Ten kinds of plasma VLCFAs were detected by gas chromatography-mass spectrometry (GC-MS), including C20:0, C20:1n-9, C20:2n-7, C20:3n-6, C20:4n-6, C22:4n-6, C22:5n-6, C20:5n-3, C22:5n-3 and C22:6n-3.RESULTS:1. There is a higher BMI,WHR, TG, LDL, FBS, FINS, HOMA-IR, FT and T in PCOS patients compared to the normal ones (P <0.05).2. There is a lower C20:4n-6/C20:3n-6 in VLCFAs of PCOS patients (P<0.05), and a higher C20:0, C20:1n-9, C20:2n-7, C20:3n-6, C22:4n-6,C20:5n-3,C22:5n-3,C22:6n-3 compared to the control group (P <0.05). In the PCOS group, patients accompanied by insulin resistance had a higher C20:0 and lower C20:2n-7,C22:5n-3 levels compared to non-resistance group (P<0.05), patients accompanied by hirsutism had a highter C20:0, C20:4n-6 and C20:4n-6/C20:3n-6 levels compared to non-hirsutism group (P<0.05).3. Multiple Stepwise Regression Analysis showed that C20:2n-7 was an independent risk factor of obesity, and C20:0 was an independent risk factor of HOMA-IR and FT level.4. Logistic regression analysis showed that C20:0 was the most significant factors related to the development of hirsutism in PCOS women.CONCLUSION:1. Disturbance of Long-chain Fatty acid metabolism existed in the PCOS patients. The main manifestations were the reduction of f C20:4n-6/C20:3n-6 and the rise of C20:0, C20:1n-9, C20:2n-7, C20:3n-6,C22:4n-6,C20:5n-3,C22:5n-3,C22:6n-3. 2. The decrease of C20:2n-7 was probably an influence factor of obesity in PCOS.3. The increase of C20:0 was probably an influence factor of insulin resistance and hirsutism and/or hyperandrogenism in PCOS.
