The Clinical Analysis of Four Internal Fixation Methods for Treating the Distal Humeral Fractures

Abstract


Objective To compare the efficacy of four internal fixation method for the treatment of distal humerus fractures . In June 2009 to December 2010 , our hospital treated 36 cases of distal humerus fracture patients . Male patients in 23 cases, 13 cases of female patients , aged between 16 to 61 years old , the average age of 36 patients by AO / ASIF classification criteria after typing fracture type is A - 6 , are B-type 12 cases, 18 cases of C - type . In all cases , the use of Kirschner wire fixation of the three cases , the use of a single plate 13 cases , 10 cases fixed by bilateral reconstruction plate , Y plate 10 cases . For this group of patients surgical approach , internal fixation and postoperative joint function recovery were analyzed . The results were followed up for 6 to 17 months , an average of 9 months , fracture healing is generally about 10 to 16 weeks , the average healing time of 14 weeks , all cases of postoperative wound infection , 1 case of fracture healing , 1 case of trauma myositis ossificans . Kirschner and bilateral reconstruction plate without loosening fracture , and the Y- plate and single- plate cases were broken . After the elbow Cassebaum scoring system , excellent in 17 cases : elbow 15 °, 130 ° elbow elbow asymptomatic . Good in 10 cases : elbow 30 ° elbow 120 ° elbow or no symptoms . 6 cases : stretch elbow 40 degrees , 90 ° to 120 ° elbow ; elbow symptoms . Poor in 3 cases : extension elbow 40 ° elbow lt ; 90 ° , excellent rate of 75.0% . Conclusion by selecting the appropriate surgical approach and internal fixation can effectively improve the therapeutic effect of distal humerus fractures . In addition, the earlier after reasonable joint function recovery exercise is also an important factor to affect the distal humerus fracture healing . Comparison , analysis and efficacy of the olecranon osteotomy way better than the other approach , because it can fully exposed distal humeral articular surface , especially for the treatment of C-type C3 fractures , have a special advantage . Can effectively avoid the triceps injury , and allow early postoperative exercise , stand joint and muscle function recovery . Four internal fixation , bilateral reconstruction plate fixed effect is excellent, due to its fixation conducive to fracture healing , reducing the incidence of postoperative complications , conducive to the early recovery of the function of the elbow .
