Objective To study the artificial liver support system on the clear effect of chronic severe hepatitis B patients with advanced inflammatory mediators high mobility group protein -1 (HMGB1), to further explore the significance of artificial liver support system in the treatment of severe hepatitis. Methods 41 cases of patients with chronic severe hepatitis B, 32 cases of plasma replacement therapy (PE), 9 cases of continuous static - venous hemofiltration therapy (CVVH), respectively, in different artificial liver support system before and after treatment, and postoperative 3 days, measured by ELISA serum HMGB1 content, comparing them before and after treatment. Simultaneous detection of the patients in each group artificial liver preoperative and postoperative total bilirubin (TBIL), change thrombin original activity (PTA). 10 cases of healthy blood donors as a control group. Results in patients with chronic severe hepatitis B serum HMGB1 content higher than the normal control group, a statistically significant difference between the two (P lt; 0.01). Serum HMGB1 content in plasma replacement in the treatment group and the continuous static - the the preoperative venous hemofiltration treatment group were 11.63 ± 3.59,14.12 ± 2.53 ng / ml, postoperatively, respectively 7.36 ± 2.78,6.30 ± 1.88 ng / ml, the two groups significantly decreased postoperative compared with preoperative, the difference was statistically significant (P lt; 0.01); therapeutically effective in patients the artificial liver postoperative HMGB1 levels below the ineffective group, the difference was statistically significant (P lt; 0.01); contrast plasmapheresis and continuous static - venous hemofiltration different treatment on the removal efficiency of serum HMGB1 prompted continuous static - venous hemofiltration plasmapheresis higher of HMGB1 clearance rate, the difference was statistically significant (P lt; 0.01 ); 3 days after HMGB1 content than after rising to compare the differences were statistically significant (P lt; 0.01), but still lower than the preoperative HMGB1 levels, the difference was statistically significance (P lt; 0.01 ). Studies have shown that plasma exchange in patients with severe hepatitis after and TBIL decline, PTA rise, the differences were statistically significant (P lt; 0.01); while continuous static - venous hemofiltration postoperative TBIL decline, compared with the preoperative difference was statistically significant (P lt; 0.05), PTA preoperative and postoperative difference was not statistically significant (P gt; 0.05) conclusion plasmapheresis and continuous static - venous hemofiltration treatment scavenging effect of HMGB1 have artificial liver treatment can clear HMGB1 inflammation factor, help to improve the prognosis of patients with chronic severe hepatitis B. Continuous static - venous hemofiltration clearance rate of HMGB1 in plasma exchange. An artificial liver treatment limited clearance of bilirubin and HMGB1.
