Abstract


Evaluation purpose: through the analysis of the clinical data, as well as some cases of acute leukemia (AL) follow-up, the region acute leukemia incidence of clinical and laboratory characteristics and clinical treatment, while looking for relief and early related risk factors and overall survival time, and then to understand the the AL onset of clinical characteristics, and prognosis of patients to provide the basis of the assessment. Methods: 347 cases collected from January 1, 2005 to 2008, 31 patients with newly diagnosed AL Hematology clinic in Shandong Provincial Hospital, in all cases in accordance with the bone marrow smears Wright-Giemsa staining morphology and cytochemistry staining characteristics of diagnosis, in accordance with the standards of the FAB classification diagnosis (specific criteria, see Chapter 34) of the South \Blood count and liver function indicators are the automatic hematology and biochemical analyzer, bone marrow cells immune phenotype Immunohistochemical detection results. Retrospective analysis of patients with the onset of age, gender, clinical manifestations, and related laboratory test results to evaluate the effect of chemotherapy for all patients, and some patients telephone follow-up, analysis of survival data, looking for prognostic factors. SPSS16.0 statistical software for data analysis, P <0.05 considered statistically significant. Results: 347 cases in patients with acute leukemia, acute myeloid leukemia (AML) 264 cases of acute lymphoblastic leukemia (ALL) 83 cases. Male and female incidence ratio of 1.22:1, and the median age of onset is 39 years old. ALL than AML more likely to have symptoms of extramedullary organ infiltration, the patient's age and the total number of peripheral white blood cells (WBC) fever, AL typing, peripheral blood platelet concentration (PLT) and the total number of WBC relevant factors affecting the bleeding . APL patients with peripheral WBC and PLT number was significantly lower than the other sub-AL patients. Constitute the normal population of patients with acute leukemia blood type differences. ALL patients more susceptible to liver damage. Myeloid leukemia cells in the patient's gender and newly diagnosed proportion affect whether early remission in patients with non-M3 AML-peripheral leukemia cells proportion of to affect ALL patients whether factors that led to early mitigation. Patients with acute promyelocytic leukemia (APL), the median survival time (33.9 months) than ALL (17 months) and AML-non-M3 (11.2 months) longer in patients with ALL group and AML-non-M3 patients survive The curves were not statistically different. AM-non-M3 patients overall survival (OS) peripheral WBC and PLT concentration, peripheral leukemia cell ratio and liver function indicators ALT risk factors affect APL patients OS, whether splenomegaly symptoms, initial peripheral blood WBC count, bone marrow after the first course of chemotherapy naive lymphocytes original proportion of affected ALL patients OS. Conclusion: AL incidence in the region in general and domestic reported basically the same. ALL than AML more susceptible to infiltration of tissues and organs, blood type and incidence of the AL there is no correlation. APL patients, the median survival time was significantly longer than the ALL and AML-non-M3 patients, ALL group survival curves of the AML-non-M3 patients there was no difference. The impact of three different genotyping AL patients with early remission and prognosis factors vary, different risk factors associated with domestic reports.
