Comparison between Surgery Followed by Chemotherapy with Surgery Only in the Treatment of Colorectal Cancer Liver Metastasis: A Meta-analysis

Abstract


Objective: To compare the effectiveness between surgery followed chemotherapy and surgery alone in colorectal cancer liver metastasis. And to analyse the safety of chemotherapy.Methods: We indentified eligible studies in Pubmed(1966-2009), Embase(1974-2009), CBM(1978-2009), Cochrane Library (Issue 4, 2009),all time to 2009.12.We selected the randomized controlled trials comparing surgery followed by chemotherapy with surgery only in the treatment of colorectal cancer liver metastasis. Effective data were extracted and Cochrane Handbook for Systematic Reviews of Interventions was used to assess the methodological quality of included studies. Meta-analysis was performed using Revman 5.0 software. Outcomes included five-year overall survival rate, liver recurrence rate, disease-free survival and safety of chemotherapy.Results: Six RCTs were identified including 580 patients. Meta-analysis showed that there was significant difference between these two groups in 5-year overall survival rate [risk ratio (RR) 1.44 (95 per cent confidence intervals (C.I.) 1.16 to 1.79),P=0.001 ],and liver recurrence rate [RR 0.60 (95 per cent CI 0.42 to 0.86),P=0.005]. We concluded that surgery followed chemotherapy was higher than surgery only in the disease-free survival. And none of the patients was die from chemotherapy. Conclusions: Surgery followed chemotherapy can raise the five-year overall survival rate and the disease-free survival rate. It can drop the liver recurrence rate and show that chemotherapy is safe .So Surgery followed chemotherapy is superior to surgery only in the treatment of colorectal cancer liver metastasis.
