Abstract


The first part: 3.0T MR dynamic enhanced MRI analysis of normal pancreas and pancreatic quantitative research purposes: the use of a 3.0T MR fast three-dimensional dynamic enhanced scanning sequence data obtained, a quantitative analysis of the normal pancreas and pancreatic perfusion studies, to explore its clinical value for the diagnosis of pancreatic cancer. Materials and Methods: 43 patients with pathologically confirmed patients with pancreatic cancer (pancreatic cancer group) and 37 patients with non-pancreatic diseases (control group) with total pancreas to LAVA nine dynamic enhanced sequence scanning, the resulting data to the ADW 4.2 workstation processing the diseased area of ??the control group were measured pancreatic head, body, tail and pancreatic cancer group and the non-diseased area 30s enhancement ratio (SER30), 90 s enhancement ratio (SER90), area under the curve (PEI), time to peak (TTP ) and the maximum enhancement slope (MSI), and the parameters within the group and between-group comparison using the SPSS 11.5 statistical software. Results: The control group pancreatic head, body and tail of the SER30 SER90, PEI, TTP and MSI differences were not statistically significant. Lesion of the pancreatic cancer group and non-lesion SER30, PEI, TTP, MSI have significant differences, SER90 does not have a significant difference. Pancreatic cancer group non-lesion control group a corresponding region SER90 and TTP statistically significant, the two groups the SER30 not have a statistically significant difference. Conclusion: perfusion differences between the different parts of the normal pancreas. The pancreatic lesion area with non-diseased area perfusion differences can reflect the cancerous tissue infiltration. TTP difference between the non-pancreatic lesion area with normal pancreas may reflect the possibility of potential lesions. The second part: 3.0 T MR body proton magnetic resonance spectroscopy analysis of the features of normal pancreas and pancreatic cancer metabolism research purposes: 3.0T MR body proton magnetic resonance spectroscopy analysis techniques to analyze the metabolic characteristics of normal pancreas and pancreatic cancer. Materials and Methods: 29 patients with pathologically confirmed patients with pancreatic cancer (pancreatic cancer group is selected; 19 cases of pancreatic cancer, pancreatic cancer, 10 cases), including 14 males and 15 females, with a median age of 55 years old; 27 cases patients with pancreatic diseases (control group), including 15 males and 12 females, with a median age of 56 years old. By GE 3.0-T magnetic resonance scanner, set on the pancreatic lesion group and non-lesion control group, the head of the pancreas and pancreatic body and tail regions of interest, parallel single voxel 1H-MRS, the resulting data transmitted the ADW 4.2 processed by the workstation, pancreatic cancer and control groups of each region of interest were measured fatty acid (FA, 5.4 ppm), the total choline complexes (t-Cho, 3.2 ppm), fats (Lip, 1.3 ppm area under the curve) and within raw water (InW, 4.7 ppm), and calculate the ROIs FA / InW t-Cho/InW and Lip / InW ratio, using SPSS 11.5 statistical software group and inter-group The resultant various ratios were compared. Results: ① The ratio of the control group, the head of the pancreas area and the pancreatic body and tail area FA / InW and Lip / InW the difference was not statistically significant, The t-Cho/InW the ratio of the head of the pancreas area smaller than the tail of the pancreas, and there is a significant difference. Of ② pancreatic cancer group lesion and non-lesion FA / InW and Lip / InW ratio significant difference; the ratio 19 cases of pancreatic cancer t-Cho/InW, pancreatic lesion less than pancreatic body and tail lesion , and there is a significant difference; 10 cases of pancreatic tail carcinoma t-Cho/InW the ratio of the difference was not statistically significant. The ③ pancreatic cancer group non-lesion corresponding to the control group, the differences were not statistically significant for all the parameters of the area ratio. Conclusion: The normal pancreas and pancreatic the 3.0T MR1H-MRS metabolic characteristics: ① metabolic characteristics of normal pancreas: t-Cho pancreatic head area is less than the pancreatic body and tail area, no significant difference in FA and Lip. ② metabolic characteristics of pancreatic cancer: FA, t-Cho and Lip reduce lesion. The ③ pancreatic non lesion with normal pancreas no significant metabolic differences, suggesting that non-diseased area is relatively normal range of surgical prompted.
