Abstract

Fecal incontinence, also known as fecal incontinence, anal sphincter without conscious control, is not aware of the capacity and nature of the rectal contents, and can not self-defecation. Loss stool Controls the ability to make people self-isolation, a serious decline in the quality of life. Completely solve the anal sphincter injuries or missing in patients with severe defecation controlled, artificial anal sphincter to be a quite promising areas of research. Urethral sphincter improved, based on the type of piezoelectric ceramic and there is insufficient memory alloy artificial anal sphincter system. Develop new types of human physiological needs artificial anal sphincter system has great significance. In this thesis, the National Natural Science Foundation of China (No.30800235), the Ministry of Education Doctoral Fund (No.20070248094) and Shanghai Jiaotong University Medical Engineering cross Fund funded projects (No.YG2009ZD103) as the basis, the combination of engineering technology and medical knowledge, to be solved in human physiology artificial anal sphincter reconstruction and its key technologies can be used to study a the clinical fecal incontinence treatment, to ensure that patients can control themselves, defecation time and eliminate the defects of the existing treatment of anal incontinence surgery, improve the patient's quality of life. This thesis is based on the mechanism of normal bowel studied in situ implantable artificial anal sphincter system is a key technology, the overall composition of the artificial anal sphincter system and system functions. The artificial anal sphincter system consists of two parts of the body devices and in vitro portable device, integrated modular system: the perception of functional modules, wireless communication module, microcontroller module, micro actuators and transcutaneous energy transmission system. Two new mini-pump: micro injection pump of the bellows and peristaltic micropump in the micro-actuator, detailing the design principle of the micro-pump. This thesis focuses on the principle of artificial anal sphincter transcutaneous energy transmission technology, analyzes and compares the receiving end of both SS and SP topology system energy transfer efficiency and maximize the efficiency of the conditions; transcutaneous energy transmission module The design of the coupling coil and the energy transmitter module and the energy receiver module design; Finally, the electromagnetic security system of the human body. This thesis through each module experiments and systems in vitro experiments to verify the feasibility of the system. First tested the wireless communication module, the transcutaneous energy transmission module and the reliability of the micro-implementing agencies. Next, simulating real human intestinal environment, from porcine intestinal effectiveness of the entire system in vitro experiments, to verify in situ implantable artificial sphincter system. Finally, the work of this thesis are summarized and proposed future direction of development, to lay the foundation for further development of a stable, reliable implantable artificial anal sphincter system.
