Clinical Research of Conservative Surgery Plus Radiation Therapy in Early-stage Breast Cancer Patients

Abstract


Objective: To study early breast conserving milk postoperative radiotherapy, side effects and cosmetic results. Methods: A retrospective analysis of 69 cases of female breast cancer patients admitted from September 2004 to October 2009, Dalian Medical University Affiliated Hospital of radiotherapy. Including 44 cases of Ⅰ, Ⅱ of 25 cases. All patients underwent breast-conserving surgery and axillary lymph node dissection. Radiation therapy to the whole breast on both sides of the tangential field irradiation, the 6MV-X-ray irradiation dose 46-50Gy, then the tumor bed the area with 6MeV or 9MeV electronic line pushed the amount of 10-14Gy total dose of 60Gy tumor bed; axillary lymph node metastasis and with 6MV the-X line and 9MeV of electronic line the mixed irradiation ipsilateral supraclavicular lymphatic drainage area, total dose of 50Gy. By the dose-volume histogram (DVH) target dose and normal tissue dose assessment; according to the RTOG / EORTC criteria to evaluate the side effects of radiotherapy; method to evaluate the cosmetic results of breast Harris; survival was analyzed using Kaplan-Meier method. Results: follow-up from the end of radiotherapy, since the deadline to March 31, 2010, median follow-up of 33 months (6-67 months) follow-up rate of 100%, 1 patient died of brain metastases, 1 patient with bone lung metastases and death cases ipsilateral supraclavicular lymph node metastasis, and to follow-up alive. Dose assessment: the target V95% of the mean is 96.63%; ipsilateral lung V20 is 17.00%; heart of the V30 was 3.44%; spinal cord maximum dose mean 0.64Gy. Radiotherapy side effects: radiation-induced skin reactions, one of 61 cases (88.4%), mild erythema, pigmentation or dry desquamation, two 8 cases (11.6%) performance as skin folds partially wet reaction 3,4 grade skin reaction; 1 radiation pneumonitis in 2 cases (2.9%); found no incidence of complications of heart damage, skin fibrosis, telangiectasia, breast and upper limb edema. Evaluation of breast cosmetic results: conserving surgery in 12 months after the evaluation of the \Survival: 1,3-year disease-free survival and overall survival were 100%; 5-year disease-free survival and overall survival were 88.2% and 93.1%; 5-year local control rate of 100%. Conclusion: 1. Early breast cancer patients with conventional radiotherapy after breast-conserving local control rate, disease-free survival and overall survival is better; radiotherapy side effects low incidence, the patient can tolerate; 3. Breast cosmetic results satisfaction, the patient's quality of life is high; strictly adhere to the premise of the breast-conserving indications breast-conserving treatment Ⅰ, Ⅱ stage breast cancer is relatively ideal treatment method; 5 for long-term cosmetic results and quality of life, but also need to be further explored and observed.
