Preoperative Evaluation of the Hepatic Vessels in Living Liver Donors: Comparison of MSCTA and 3D-DCE-MRA

Abstract


Objective:To determine whether MSCTA and 3D-DCE-MRA will be able to perform a complete and accurate role in preoperative evaluation of the hepatic vascular anatomy for living liver donors.Methods:Thirty living donor candidates underwent preoperative MSCTA and 3D-DCE-MRA. Data analysis was performed by 4 reviewers (2 radiologists and 2 surgeons), based on source images and 3-dimensional postprocessing. Determination of image satisfaction degree was based on 4 grades (1=nondiagonostic,2=fair,3=good,4=excellent). They should also count the numbers of aberrant vessels, including non-Michels I type, segment IV branch from right hepatic artery, trifurcation of portal vein branches, right back inferior hepatic vein and segment VIII branch of hepatic vein draining to middle hepatic vein.6 donors suffered living donor liver transplantation. Surgical findings were used as a standard of reference and then statistical analysis was performed.Results:Both techniques are proved to be efficient to evaluate hepatic artery and portal vein for potential living donors. There was no significant difference between the two. The main advantage of 3D-DCE-MRA lies in the ability to assess the hepatic vein systems more accurately. There was a significant difference between the two.3D-DCE-MRA could be the first choice to assess potential living donors’vessels.
