Abstract

ObjectiveAcute Pancreatitis is a kind of common clinical surgical disease, the disease drastically occurs and its mortality rate is higher,therefore,it was widely concerned, the disease is not a simple mechanism of pancreatic enzyme digestion, at present their focus aim at the pancreas microcirculation gradually.Nitric oxide(NO)is compounded in the blood vessels and nerves endothelial cells, it involves the pancreas function and changes in blood flow, and inducible NO synthase impacts on the activity of NO.In recent years leptin or arginine unilateral factor impacts acute pancreatitis and have certain effect.Arginine as the semi-synthesis of amino acids is mainly compounded in liver cell, vascular endothelial cells and macrophages. In metabolic process, nitric oxide synthase can induce NO with activity.Adequate nitric oxide can expand blood vessels, inhibit cohesion of platelet and reduce damage of the oxygen centered free radicals to protect the micro-vascular endothelial cells to further damage.Leptin is a kind of protein hormone. It uses aimed receptors on the membrane and the corresponding signal transduction system to play a role. It has a variety of regulating way, when leptin receptor combine with ligand, JAK phosphorylates, then make STAT3 phosphorylated to t release ocalcium ionic iNOS,express NO and promote inflammation to be released and producecytotoxic effects.There is a kind of difference that leptin can also make use of cNOS of dependence of synthesis of calcium ions to play a role.Leptin can excite sympathetic nerve,dilate blood vessels and reduce the negative effect caused by the formation of the excess NO expression.According to the function of leptin and arginine, the combination of leptin and arginine cure acute pancreatitis of serum amylase, the formation of nitric oxide and the expression of inducible nitric oxide synthase. At last the combination can protect pancreatic damage.Methods1. Using immunohistochemical method observe the iNOS expression.2. Making use of Nitrate reduction enzymatic method detect pancreas NOcontent.3. Pancreas morphology detection and under the microscopic inspection assessing pancreas tissue damage4. pancreatic serum amylase content determination.Results 1. pancreatitis rats in the pancreas,nitric oxide of the combined group of leptin and arginine,arginine group and leptin concentrations are less than acute pancreas group,the pancreas tissue pathology scores were lower than acute pancreas group,pancreatic serum amylase value also are lower than acute pancreas group, the combination group is obviously decreased.2. nitric oxide synthase immunohistochemical section at high magnification shows that negative control group display negative expression or weak positive expression,arginine group showed positive expression, leptin group display positive expression, combined group show weak positive expression.ConclusionResearch results show that the combination of leptin and arginine can obviously improve the curative effect of acute pancreatitis, rather than a single therpy using leptin or arginine, the combination of leptin and arginine dramatically reduce activity expression of inductive nitric oxide synthase, reduce nitric oxide.The result can include①reducing iNOS expression and reducing catalytic excessive NO,for example,reducing the toxicity of cells and reducing inflammation,such as cutting oxygen free radicals,inactivating oxygen free radicals,which reducing the negative influence on oxygen free radicals and strengthening liquidity cells, improveing microcirculation deformation;②reducing the iNOS expression and increases cNOS to produce NO,the NO can improve pancreas microcirculation③promoting excitement of sympathetic nerve and reducing iNOS expression to result in nutric effect.Through the above ways,to improve the microcirculation, reduce pancreatic tissue damage and prevent the development of acute pancreatitis,the combination of confirmed leptin and arginine can obviously improve protect acute pancreas.
