PVMM the treatment of gestational trophoblastic tumor clinical analysis of 26 cases

Abstract


Objective: To investigate PVMM program ( carboplatin, etoposide , methotrexate and mifepristone ) treatment of gestational trophoblastic tumor efficacy and safety . Methods : A retrospective analysis from December 1998 to February 2008 , Shandong Provincial Hospital PVMM chemotherapy for 26 cases of malignant trophoblastic tumor ( invasive mole and choriocarcinoma ) clinical data , including 23 previously untreated patients cases, two cases of drug-resistant patients , patients with recurrent one case . 26 patients in the surgical treatment of seven patients supplemented observe the program's efficacy and toxicity. Results: 26 cases of GTN patients received a total of 100 courses of PVMM chemotherapy , the average number of courses 3.8 , the overall complete response rate was 96.2% ( 25/26 ) . Among them, 13 cases of invasive mole patients , the total number of courses 46 , the average number of courses 3.5 are complete remission , complete remission rate was 100% ; 13 cases of choriocarcinoma patients , the total number of courses 54 , the average treatment number of 4.2 , complete remission rate was 92.3% ( 12/13 ) . 23 cases of treatment -naïve patients the total number of 85 , the average number of courses 3.7 , 22 cases of complete remission , complete remission rate was 95.6% ( 22/23 ) , one case occurred after drug treatment to the outer court ; 2 cases of patients resistant to chemotherapy total number of courses 12 ( not including the initial application of 5-FU KSM resistance occurred in four courses ) , applications are complete remission after PVMM ; 1 case of relapse for the application of 5-FU KSM 4 cycles of remission relapsed after three months , given four PVMM complete remission after treatment . The program is mainly toxicity was myelosuppression , gastrointestinal reactions and alopecia , myelosuppression , both Ⅰ - Ⅲ degree , is not fatal toxicity. Conclusion : PVMM program high cure rate can be used as a safe, effective chemotherapy regimen for patients with malignant trophoblastic tumor treatment, other drug resistance or relapse cases satisfactory results can be obtained . Mild side effects of chemotherapy , patients tend to accept , it is worth further study.
