The Subtypes of Renal Cell Carcinoma: A Retrospective Study of Clinical Features and CT Appearances Related to Pathological Results

Abstract


Objective: The clinical and CT appearance, and the results of pathology with renal cell carcinoma(RCC) were analyzed and discussed retrospectively in this study,and explore the feasibility of making a diagnosis of the subtypes of RCC according to the features of routine examination.Materials and Methods: 152 patients with pathologically proved RCC were analyzed retrospectively. All patients were examined with spiral CT in plain and multiphase enhancement scans before operation. All clinical symptoms、CT appearance and pathological findings were analyzed by statistic software(SPSS13.0). Results: 122 patients of renal clear cell carcinoma were common in elderly men. Haematuria and anemia were founded in 28.4% and 23.0% of them respectively. 79.5% showed heterogeneous intensity on conventional CT and 90.2% were significantly enhanced on Incremental Dynamic CT. In 13 patients of papillary renal cell carcinoma,11 of them(84.6%) were diagnosed by hematuria, and 61.5% showed anemia. On conventional CT, 92.3% showed heterogeneous intensity.61.5% and 30.8% respectively showed longitudinal and moderate enhanced image on Incremental Dynamic CT. 7 patients were diagnosed of Chromophobe cell renal cell carcinoma. They all had no obvious symptoms.All tumor textures were of uniformity on conventional CT and longitudina enhanced on Incremental Dynamic CT.The significant differences( p<0.01) of CT appearance、clinical symptoms and routine examination were found between clear cell RCC and chromophobe cell RCC. So were clear cell RCC and papillary RCC.Conclusions: There were some different features in the CT appearance and chemical examinative results of the renal cell carcinoma subtypes, which were helpful for preoperative diagnosis in the subtype of RCC, Selection of surgical procedure and determine the scope of the lymph node dissection. CT staging also may be an appropriate diagnostic methods for staging before operation.
