Effect of 6%HES130/0.4 Proloading on Blood Gases of Umbilical Core in the Parturient for Cesarean Section

Abstract


Objective To observe the effects of pre-anesthetic volume expansion with 6%hydroxyethylstarch (HES130/0.4) on fetal blood gas values in nulliparous parturient undergoing epidural anesthesia for elective cesarean section. Methods Sixty healthy parturients in First Affiliation Hospital of Xinjiang Medical University scheduled for elective cesarean section during epidural anesthesia were randomly allocated into two groups:each of them receiving hydroxyethylstarch solution 6%(HES130/0.4)(n=30),and lactate ringers solution (n=30), respectively. All the parturients were infused with hydroxythyl starch or lactate ringers solution with the velocity 15ml·kg-1·h-1.After 30 minute the epidural anesthesia were administrated .The changes of blood pressure ,heart rate and oxyhemoglobin saturation were monitored. Meanwhile comparisons of these umbilical cord venous and artrial blood gas variables were made between two groups. After that, Apgar scores of the new-borns were recorded. Results Demographic data including age, sex, weight ASA status and anesthetic duration. were similar among the groups. The PvO2 of umbilical cord venous blood in group I is significantly highest than that in group II (P<0.01).The umbilical cord venous PvCO2 was lower in group I(P<0.05). whereas PaO2 was higher in group I(P<0.05) than in group II.There was no difference between two anesthetic groups in pH measure. That means There is more oxygen which can be used by fetus. Neonatal Apgar scores were equally ten at five minutes after delivery. This studies have shown no effects of preloading on Apgar. Conclusion Pre-anesthetic volume expansion with hydroxythylstarch 6% (HES130/0.4) can not only maintain stable hemodynamics during epidural aesthesia but also efficiently improve tissue perfusion of blood ,microcirculation and uteroplacental blood flow, simultaneously increase supplement of oxygen to the fetus.
