Objective: To investigate the complex surgical indications for lumbar spinal stenosis , choice of surgical procedure and Comparative efficacy . Methods: A retrospective analysis of our hospital orthopedic admitted from January 1996 to January 2008 , 67 patients with complex lumbar spinal stenosis patients clinical data : age 34-76 years, mean 59.2 years, duration of 4 months -30 years , an average of 5 years 2 months ; 56 patients were followed up for laminectomy decompression 27 cases , laminectomy and interbody fusion pedicle screw system fixation in 29 cases . Results: 1. According to the JOA score , two surgical postoperative excellent rate : 59.26% , 89.66% , and the difference was significant . ( P lt; 0.05 ) based on X-ray examination of the lumbar dynamic lumbar instability incidence were : 66.67% , 6.90% , and the difference was significant . (P lt; 0.05) 3. Two surgical incidence of postoperative complications were : 7.41% , 14.81% . Conclusion: For lumbar spinal stenosis with lumbar spinal instability in the vacuum line interbody fusion internal fixation can significantly improve its stability . For complex lumbar spinal stenosis underwent laminectomy decompression and interbody fusion and internal fixation of pedicle screw system the best therapeutic effect . 3 . Strict surgical indications , selected according to the different types of lesions of different surgical procedures .
