ObjectiveThe experiment was to obtain an ideal analgesia project with the characteristics of patient satisfied, less side-effect and high safety from the analysis and comparison of the data collected from the postoperative patients of the mixed hemorrhoids, which were respectively treated by balanced analgesia, locally injecting long-term analgesic injection, and PCA at the end of the operation.MethodThe patients, who were suitable for the selective standards and willing to accept the experiment, were divided into three groups according to their admission sequence which are balanced analgesia group (group A), local analgesia group (group B) and PCA group (group C). There are ten cases in each group. All of them had the surgery of internal ligation and external excision of mixed haemorrhoids under the sacral anesthesia. At the end of each operation in group A, firstly, we injected compound lidocaine hydrochloride injection into the infra-area, and the edge of the wound-surface, and the ligation area of the interal haemorrhoids. Secondly, we wrapped two layers of sterilized dressing around the lateral-aperture duct for one time, then inserted it into the anal canal with the external orifice outside which was connected with the patient controlled pump which’s filled with 2.39mg/ml ropivacaine mesylate injection.For the patients in group B, they had the same treatment as the first step in group A. For the patients in group C, they had the same treatment as the second step in group A at the end of the surgery. Observe and record each patient’s conditions such as postoperative pain, whether or not they have urine retention, etc. we dealt and analyzed the collected data with statistic methods.ResultsThe operation time compare of the three groups showed P>0.05. The 2-48h VAS average score for group A, group B and group C was0.92±0.35,3.91±0.51 and 3.73±0.75 respectively (P<0.01).The first-defecation VAS average score was 1.59±0.34,3.98±0.75 and 3.86±0.59 respectively (P<0.01).As for the condition of urine retention, we got P<0.05, and the swollen edge of the wound, P>0.05.ConclusionThe balanced analgesia has a good therapeutic effect on the postoperative pain in the patients with mixed haemorrhoids. The balanced analgesia has the effect of relieving the postoperative urine retention for the patients who suffer from mixed haemorrhoids. The balanced analgesia is a practicable, effective and safe analgesic project.
