Study of the Visual Pathway with MRI on Patients with Primary Glaucoma

Abstract


Objective: To probe into the regional variation trend of the primary glaucoma visual pathway and the relevance among visual pathway changes, clinical classification and severity illness scale.Methods: Twenty participants from age 24 to 72(10 males, 10 females,average age: 50.90±14.69) of binoculus primary glaucoma patients were recruited to join this study. According to clinical pathology classification, two groups are divided: one is primary open angle glaucoma(POAG) and the other is primary angle-closure glaucoma(PACG). All research subjects shall accept diffusion tensor imaging (DTI) and proton density weighted image (PDWI) scans, and their fractional anisotropy (FA) and average diffusion coefficient (DCavg) values of optic nerve, optic tract and optic radiation, and the volume and height of their lateral geniculate body (LGN) shall be manually measured. Then to compare with the testing results of 18 healthy volunteers (8 males and 10 females, age range: 25-69, average age: 45.7±15.3) of the same index.SPSS17.0 was used to analysis the changes of primary glaucoma patients’visual pathway and the differences between different primary glaucoma pathological groups using software. p<0.05 is statistically meaningful.Results: In POAG group,the FA values of optic nerves on the left and right side decrease obviously(p<0.001、p <0.001), the DCavg values increase obviously(p =0.01、p =0.003);the FA values of optic tract on the left and right sides decrease obviously(p <0.001、p <0.001), the DCavg values increase obviously(p <0.001、p <0.001);the FA values of optic radiation on the left and right sides decrease obviously(p <0.001、p <0.001), the DCavg values has no significant difference(p =0.328、p=0.270);the volume of LGN on the left and right sides reduce obviously(p<0.001、p<0.001), its height decrease obviously(p<0.001、p=0.002).In POAG group,the FA values of optic nerve on the left and right sides decrease obviously (p<0.001、p<0.001), the DCavg values increase obviously(p=0.043、p=0.029);the FA values of optic tract on the right and left sides decrease obviously(p<0.001、p<0.001), DCavg values increase obviously(p=0.004、p=0.017);the FA values of optic radiation on the left and right sides decrease obviously(p<0.001、p<0.001),the DCavg values increase obviously(p=0.004、p=0.017);the volume of LGN on the left and right sides reduce obviously(p<0.001、p<0.001), its height decrease obviously(p<0.001、p<0.001).The right LGN height of POAG and PACG is statistically different(p=0.016), the LGN value of POAG is a little higher(4.267±0.646mm);the FA value, DCavg value, bilateral LGN volume and the left LGN height of optic nerve, optic tract and optic radiation have no significant statistically differences(p>0.05).Conclusion: The study confirms that varying degrees of degenerative changes happen to optic nerve, optic tract, optic radiation, and lateral geniculate body of both POAG and PACG. The changes can be surveyed with MRI and there is no statistical difference on pathway changes of the two groups.
