Analysis of the Long Term Complications of Patients Suffered from Subtotal Gastrectomy Due to Benign Gastroduodenal Diseases

Abstract


The purpose is to understand due to benign diseases of the stomach, duodenum gastrectomy surgery in patients with long-term complications and quality of life , analysis of its relations with gastrectomy surgical approach . Methods 116 cases of stomach , duodenum benign disease in patients over most of the 10 years after resection of gastric for the study group , 80 cases of healthy control group . Using quality of life the Digestive Disease Quality of Life Index (GIQLI) assessment study group and the control group . Including endoscopy, the nutritional status of the follow-up of the patients of the study group , and to assess its long-term complications , and analyze its relationship with the surgical approach . The results of the study group digestive disease quality of life index score compared with the control group ( 102.5 ± 10.2 vs 118.3 ± 8.1 , P lt; 0.05 ) , serum folic acid, vitamin B12, iron protein , albumin water an average lower than those in the control group (P lt ; 0.05 ), anemia , osteoporosis incidence rate was higher (P lt; 0.05). Study group 116 patients endoscopic remnant gastritis (100%) in varying degrees , associated with bile reflux in 49 patients ( 42.2% ) , the anastomotic ulcer 20 cases (17.2 %) , Helicobacter pylori infection in 43 cases (37.1 % ) , gastric remnant primary carcinoma in 5 cases (4.3%) ; anemia in 21 cases ( 18.1% ) , osteoporosis, 29 patients (25.0 %) . Which , the incidence of Billroth Ⅱ - surgical patients with residual stomach and anastomotic ulcer of than Billroth Ⅰ type is low ( 12.8% vs 23.9 % , P lt ; 0.05 ) , while Billroth Ⅱ - bile reflux occurrence rate than Billroth Ⅰ type is high ( 48.6% vs 23.6%, P lt; 0.05), the remaining residue gastropathy and complication rate differences had no statistical significance ( P gt ; 0.05) . Conclusion gastroduodenal benign diseases gastrectomy surgery for more than 10 years , post-operative complications , including bile reflux remnant gastritis , the residual gastric ulcer , gastric stump cancer , malnutrition , etc. ; gastroduodenal benign diseases row gastrectomy surgery for more than 10 years the quality of life than the healthy control group significantly decreased ; Billroth - I remnant gastric ulcer incidence was significantly higher than the Billroth Ⅱ bile reflux incidence was significantly lower than Billroth Ⅱ , two surgical methods H. pylori infection had no significant effect ; bile reflux subtotal gastrectomy gastric remnant cancer becomes one of the important reasons .
