Abstract


Through more than thirty years of empirical and clinical studies, the knowledge of ARDS is known more and more, and all kinds of therapeutic measures have been taken to decrease the ARDS mortality. However, ARDS morbidity and mortality are still high clinically, which is not resolved yet. At present, there are lots of studies on single predicting marker of ARDS, but the studies on predicting ARDS occurrence using different markers from multiple aspects are scarcely reported in documents. Through observing and analyzing the changes of peripheral blood SP-A and CRP levels in severe multiple injury patients, the author tries to discuss the correlation between ARDS and them, thereby to provide some theoretic proofs for early forecasting ARDS caused by trauma and taking effective therapeutic measuresObjectiveThe primary objective of the study is to discuss the correlation and its clinical significance between the ARDS caused by trauma and the changes of peripheral blood SP-A and CRP levels in severe multiple injury patients.Materials and methods81 severe multiple injury patients (ISS≥16) were divided into ARDS group (14) and nonARDS group (67); 20 minor injury patients (ISS<16); 10 health volunteers served as the control group. Semi-random control trial was taken. Peripheral blood in health volunteers was collected once; peripheral blood in multiple injury patients and minor injury patients was collected on the days of 1, 3 and 7 after trauma; it was centrifuged at the speed of 3000 r/min, then immediately stored in a -20℃icebox. At the same time, femoral artery blood was drawn and blood gas analysis was performed; the datum of FiO2 was recorded simultaneously, then Oxygenation index was calculated according to the ratio of PaO2(mmHg) and FiO2. The change of Plasma SP-A level was qualitatively or semi-quantitatively analyzed by Western Blot technique; the concentration of serum CRP was measured by Sandwich ELISA medthod (SP-A monoclonal antibody and CRP ELISA
