Abstract


Stress urinary incontinence (SUI) is a disease of involuntary loss of urine caused by increasing abdominal pressure without contraction of cystic bladder muscle. That is a social or hygienic problem which exact mechanism and basic pathophysiology are poorly understood.Collagen is the main content of pelvic connective tissue which is very important in all tissues which control micturition. Previous studies concerned about alternation of collagen content in incontinent women. However, the changes of collagen subtype are still controversial. Accommodation of collagen depends on different factors. Matrix metalloproteinases (MMPs) and Tissue inhibitor of metalloproteinases (TIMPs) are the most important proteinases in accommodation of metabolism of collagen. MMPs is a family of structurally related proteins which degrade extracellular collagen .The activity of MMPs can be regulated by TIMPs with a mixture ratio of 1:1. So activity of MMPs is down-graded and degradation of collagen is decreased.This study was designed to investigate immunohistochemical expression of collagen type I and typeⅢ,matrix metalloproteinases (MMP-1), and its inhibitor, the tissue inhibitors of metalloproteinases(TIMP-1) in pubocervical fascia from the women with stress urinary incontinence. The biochemical changes in pubocervical fascia was illustrated. And relationship between collegan metabolism and happening of stress urinary incontinence.Material and methods:A group of 24 patents with SUI who had undergone surgery during Sep.2004 to Mar 2006 was chosen as the study group. (SUI group,9 premenopausal patients,,15 postmenopausal patients ). The control group was chosen from a group of women who had undergone hysterectomy for benign gynecologic disorders without SUI or POP. (9 premenopausal patients, 15 postmenopausal patients ). Between the two groups ,there was no significant difference in age,parity,BMI and time of menopause(p>0.05),so the two
