Effects of Sufentanil Target-controlled Infusion on Thromboxane A2 and Prostacyclin in Patients Undergoing Laparoscopic Cholecystectomy

Abstract


In this study, by comparing target - controlled infusion of sufentanil and fentanyl total intravenous anesthesia in laparoscopic cholecystectomy , explore the impact and significance of thromboxane A2 , prostacyclin concentrations . Select ASA Ⅰ - Ⅱ level in patients undergoing elective laparoscopic cholecystectomy 20 cases were randomly divided into sufentanil and fentanyl groups , with 10 cases in each group . Set sufentanil and fentanyl effect-site target concentration were of 0.4ng/ml and 4ng/ml composite total intravenous anesthesia . Respectively before anesthesia , before pneumoperitoneum , pneumoperitoneum after 20 min, and lift pneumoperitoneum 10min four de facto non-invasive monitoring of systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate ( HR ) , at the same time venous blood 3 ml of plasma thromboxane B2 (TXB2) were measured by radioimmunoassay and 6 - keto - prostaglandin F1α (6-Keto-PGF1α) content . The results showed that pneumoperitoneum after 20min , sufentanil group SBP, DBP is significantly lower than the fentanyl group ; compared with those before anesthesia , the two groups after pneumoperitoneum 20min TXB2 were significantly increased 10min after the lifting of pneumoperitoneum decline , still compared with the fentanyl group was significantly higher than that before anesthesia ; the sufentanil group pneumoperitoneum after 20min, the lift pneumoperitoneum 10minTXB2 content decreased significantly ; sufentanil anesthesia group was significantly higher than before anesthesia 6-Keto-PGF1α reached a peak after pneumoperitoneum 20min ; lift pneumoperitoneum after 10min, sufentanil group 6-Keto-PGF1α fentanyl group was significantly higher compared ; the fentanyl group of pneumoperitoneum after 20min, lifted the pneumoperitoneum after 10min plasma TXB2/6-Keto-PGF1α ratio compared with those before anesthesia significantly increased in comparison with fentanyl , sufentanil anesthesia each time point TXB2/6-Keto-PGF1α ratio significantly decreased . Therefore, can be inferred from the results of laparoscopic surgery thromboxane A2 , prostacyclin release increased ; sufentanil pneumoperitoneum caused inhibition of the stress response of the cardiovascular system is stronger than fentanyl ; with fentanyl phase than sufentanil to the maintain the relative balance of laparoscopic surgery plasma thromboxane A2, prostacyclin , improve microcirculation and perfusion of the organ protective effect .
