Abstract


Background and purpose of allergic purpura is one of the most common pediatric vasculitis, in recent years, the incidence increased year by year, the exact pathogenesis is not yet clear. Generally believed involved in the pathogenesis of IgA and IgG-mediated allergy type III. But in recent years, the clinical findings of IgE-mediated type I allergy may be involved in the part of the incidence of allergic purpura. CD23 and CD19 are close relations with IgE factor. This article by detection of allergic purpura nephritis and peripheral blood lymphocyte expression of CD23, CD19, and explore the CD23, the relationship between the role of CD19 in the pathogenesis of children allergic purpura nephritis (HSPN), CD23, CD19. Method 1. Using flow cytometry and enzyme-linked immunosorbent assay (ELISA) to detect 30 cases of allergic purpura nephritis (HSPN) in children and 30 cases of allergic purpura peripheral blood of children with CD23 positive (CD23 -, CD19, CD19 positive lymphocytes to ) percentage and serum IgE levels. And 30 healthy children as control. 2.30 cases of allergic purpura nephritis (HSPN) in children 27 cases of renal tissue in B-positioning downstream percutaneous renal biopsy, taken by light microscopy, electron microscopy, immunofluorescence checks. 3. Application SPSS10.0 statistical software for data management and statistical analysis. The inspection level α = 0.05. Results 1. Allergic purpura (HSP) Group allergic purpura nephritis (HSPN) in peripheral blood CD23 positive, CD19-positive lymphocytes (CD23 ~, CD19 ~) percentage and serum IgE levels were higher than the normal control group (P <0.01) ; allergic purpura nephritis (HSPN) group the percentage of CD23 positive lymphocytes (CD23 ~), the average percentage of CD19 positive lymphocytes (CD19 ~) and serum IgE levels higher than the allergic purpura (HSP) group (P <0.01) . 2. Allergic purpura nephritis group of 30 patients, 27 cases renal biopsy biopsy of renal pathological grade Ⅱ grade 18 cases, Ⅲ grade and above nine cases, renal pathology III level and above CD23-positive lymphocytes (CD23 ~) percentage, the percentage of CD19 positive lymphocytes (CD19 ~) above the renal pathological grade Ⅱ group (P <0.01). 3. Pearson correlation analysis found that children with allergic purpura, peripheral blood CD23 ~ lymphocytes percentage of serum IgE, CD19 to lymphocyte percentage was positively correlated (correlation coefficients were 0.322,0.336, P less than 0.05, 0.01); CD19 to lymphatic The percentage of cells with serum IgE was no correlation (correlation coefficient of 0.165, P> 0.05). Conclusion 1. Allergic purpura (HSP) in children with peripheral blood CD23 to CD19 to lymphocyte percentage and serum IgE levels there is a significant difference compared with normal controls, and CD23 with IgE, CD19 positive correlation between CD23, CD19, and IgE may jointly involved in the occurrence and development of allergic purpura, all three may play a synergistic or additive. 2. Anaphylactoid purpura nephritis group (HSPN) CD23 positive (CD23 ~~) and CD19 positive (CD19 ~) lymphocyte percentage is higher than the allergic purpura group, and the higher classification of renal pathology, peripheral blood lymphocytes CD23, CD19 expression of the higher rate, the difference was statistically significant. Detection of peripheral blood lymphocyte CD23, CD19 levels can be used as the indicator of the severity of allergic purpura nephritis kidney disease. 3. CD23 can be used for indirect evaluation of allergic purpura patient's immune status. 4. Detection in peripheral blood CD23 CD19 to lymphocyte percentage and serum IgE levels in allergic purpura condition judgment has some value, but needs further study.
